Parkinsons can impact sleep in a number of ways, ranging from trouble falling or staying asleep at night to excessive sleepiness during the day. A good sleep boosts everything from your mood to your ability to think and process to your physical movement. Understanding sleep problems and Parkinsons is often the first step you can take to enhance your sleep.

Parkinsons can affect sleep in many different ways, including trouble falling or staying asleep, vivid dreams, waking up frequently during the night and excessive sleepiness during the day. Like other non-motor symptoms, sleep problems can appear before the more recognized motor symptoms, like tremor or stiffness.

People with Parkinsons typically experience some combination of insomnia and sleep fragmentation . Studies have shown people with Parkinsons have different sleep patterns and that their deepest periods of sleep during the night are shorter and interrupted more often than people without Parkinsons. Often this is made worse by medications that may wear off during the night, causing painful stiffness, difficulty moving in bed or other symptoms to return and disrupt sleep.

Anxiety Depression And Other Psychological Issues

Anxiety, depression, apathy and other psychological symptoms of Parkinsons can significantly impact your quality of sleep. Have a conversation with your doctor if you are experiencing these or other psychological effects of Parkinsons. Improving these symptoms will not only enhance your overall quality of life, but it may also help you sleep better too. In addition, some medications for these types of issues can cause drowsiness which may improve sleep quality, particularly if taken at night.

What Happens In Pdd

People with PDD may have trouble focusing, remembering things or making sound judgments. They may develop depression, anxiety or irritability. They may also hallucinate and see people, objects or animals that are not there. Sleep disturbances are common in PDD and can include difficulties with sleep/wake cycle or REM behavior disorder, which involves acting out dreams.

PDD is a disease that changes with time. A person with PDD can live many years with the disease. Research suggests that a person with PDD may live an average of 57 years with the disease, although this can vary from person to person.

How Is Parkinson Disease Diagnosed

Parkinson disease can be hard to diagnose. No single test can identify it. Parkinson can be easily mistaken for another health condition. A healthcare provider will usually take a medical history, including a family history to find out if anyone else in your family has Parkinsons disease. He or she will also do a neurological exam. Sometimes, an MRI or CT scan, or some other imaging scan of the brain can identify other problems or rule out other diseases.

  • Consider making environmental adjustments to protect the person with RBD and bed partner from injury. This may include padding the floor, creating a barrier between bed partners, or sleeping in separate beds or rooms.

  • Talk to your doctor about the over-the-counter sleep aid melatonin. Doses up to 12 mg one hour before bedtime can improve RBD symptoms.
  • Clonazepam has been shown in large case series to improve RBD in 80 to 90% of cases. It is often used when melatonin is not effective. The dose of clonazepam required is low, usually from 0.5 mg to 1.0 mg. The adverse effects of clonazepam include nocturnal confusion, daytime sedation and exacerbation of obstructive sleep apnea, if present.

    Parkinsons disease is a progressive neurodegenerative disorder characterized by the loss of dopamine-producing nerve cells in the brain. Dopamine is a cell-signaling molecule that relays information between nerve cells and between the brain and the muscles.     Be prepared to explain when sleep disruptions happen and how they affect your life. Keeping a sleep journal for a few weeks can help you remember the details.

    If your provider suspects you may have a sleep disorder, they may recommend you have a sleep study. This overnight test uses electrodes attached to your skin to track how your body functions when youre sleeping.

    How Can We Support The Sleep/wake Cycle Of Pdd

    For people with PDD who are confused about the day-night cycle, some daily strategies can be helpful. At night, starting a lights out routine that happens at the same hour every day, where all curtains are closed and lights are turned off, can help the person understand that it is sleep time. During the day, opening the curtains, allowing the person with PDD to spend as much time in the daylight as possible, avoiding naps, and organizing stimulating activities, can be helpful. Having lots of calendars and clocks in every room might also help a person with PDD be less confused about the time of day.

    Why Does Cold Weather Affect Parkinsons Symptoms

    There are several reasons why cold weather impacts people with Parkinsons. One of the nonmotor symptoms of Parkinsons disease involves problems regulating the autonomic nervous system. Your autonomic nervous system controls many functions, including body temperature regulation.

    Some people diagnosed with Parkinsons have autonomic dysfunction, which makes them sweat too much or not enough. Autonomic dysfunction can cause problems in cold weather because your body may not be able to keep you warm enough. People with Parkinsons may feel colder than those around them when the temperature drops, which can lead to symptoms like uncontrolled shivering. Some find that the shivering triggers an increase in their tremors, which makes them shake even more.

    Some medications for Parkinsons may also bring on greater temperature sensitivity or problems with sweating. These side effects may be worse when you are first starting a new medication or when tapering off a medication.

    Drugs Used To Treat Parkinsons Disease

    The common Parkinsons disease medication of carbidopa and levodopa can contribute to insomnia for some people, says Horvat. Thats because its replacing the dopamine that youve lost in Parkinsons disease, she says. Research has shown that dopamine receptors play a role in wakefulness.

    Sometimes when people are first started on this medication, they will take a dose right before bed rather than closer to their dinner, says Horvat. Then theyre not able to get to sleep because the dopamine affects the reward center in the brain and gives people a high, she says.

    Besides timing the medication further from bedtime, your doctor may suggest taking an extended-release capsule of carbidopa and levodopa, says Horvat. That has a lower peak dose effect, so the stimulation is milder, and it lasts a little longer. It can allow patients to have more of a baseline rather than a peak at night, which can cause the insomnia, she says.

    If you suspect your medication is making sleep difficult, tell your doctor dont stop taking the medicine as prescribed, says Horvat. Sometimes we can time the medication in a different way or in some cases we do change the medication but this is not something to try to figure out on your own.

    What Is A Parkinsons Tremor

    Other health issues can also cause tremors, like multiple sclerosis or essential tremor. But Parkinsonâs tremors are different because theyâre usually:

    • Resting. Parkinsonâs tremors happen when your muscles are still. They go away when you move. They also lessen while you sleep. For example, if youâre sitting in a chair with your arm relaxed, you may notice that your fingers twitch. But if youâre using your hand, like when you shake someone elseâs hand, the tremor eases or stops.
    • Rhythmic. Parkinsonâs tremors are slow and continuous. They arenât random tics, jerks, or spasms.
    • Asymmetric. They tend to start on one side of your body. But they can spread to both sides of the body.

    Eating Swallowing And Saliva Management

    Some people with Parkinsons may find they have difficulties with eating, swallowing and saliva control at some stage of their journey with Parkinsons.

    Parkinsons can cause the muscles in the jaw and face to become stiff which affects the control of chewing and swallowing.

    Another symptom of Parkinsons can be producing excessive saliva. The stiffer facial muscles can change the nature of saliva, which may become thicker and stickier.

    Insomnia And Trouble Falling Asleep Or Staying Asleep

    Some survey respondents said they have a hard time falling asleep, while others have trouble staying asleep. Another group of respondents has trouble with both sleep issues. Research shows that more than 35 percent of people living with PD have insomnia.2

    Several respondents shared their experiences with insomnia and trouble staying asleep:

    I have no problem falling asleep, but after 3 hours I am fully awake for another 4 hours before I go back to bed. I have never had sleep issues of any kind in the past.

    I have a hard time getting to sleep and staying asleep. I am tired but cannot sleep. I fall asleep in the middle of doing other things.

    How Is Parkinsons Disease Diagnosed

    There is no test that can prove that you have PD. The diagnosis is based on you having the typical symptoms . In the early stage of the disease, when symptoms are mild, it may be difficult for a doctor to say if you definitely have PD. As the symptoms gradually become worse, the diagnosis often becomes more clear.

    PD is sometimes confused with other conditions. Some conditions can give Parkinsonism features that is, symptoms similar to PD but caused by other conditions. For example, some medicines used to treat other conditions can cause side-effects which resemble symptoms of PD. Some rare brain disorders can also cause similar symptoms.

    Therefore, it is normal practice in the UK to be referred to a specialist if PD is suspected. The specialist will be used to diagnosing PD and ruling out other causes of the symptoms. They will usually be either a neurologist or a doctor specialising in elderly care. If there is still doubt about the diagnosis, sometimes a scan of the brain is carried out. This helps to differentiate PD from some other conditions that can cause Parkinsonism features. Other tests sometimes needed include blood tests and tests of your sense of smell.
