There is no medical test that can confirm a diagnosis of ET. Your physician will review your medical history and family history, and will conduct a physical exam. Unfortunately, no. Parkinsons disease is long-term disease that worsens over time. Although there is no way to prevent or cure the disease , medications may significantly relieve your symptoms. In some patients especially those with later-stage disease, surgery to improve symptoms may be an option.

How Can I Reduce Tremors

In the early stages of Parkinsons disease, some people can reduce tremors by pressing or rolling a ball, pen or other small object. Tremors can also increase during stressful situations, during which you should take some time to breath and relax.

Tremors are more difficult to control during the more advanced stages of the disease. Here are some ways to better manage your tremors if they interfere with daily activities:

  • Write on a keyboard rather than by hand
  • Use speech-to-text cell phone apps
  • Drink with a straw
  • Use heavier utensils. If this does not help, you can purchase electronic utensils designed to counter your tremors
  • Purchase clothing and shoes that are easy to put on

  • Entrances and doorways: Install metal handles on the walls adjacent to the doorknobs of all doors to make it more secure as you travel through the doorway.

Parkinson’s disease is marked by a progressive loss of brain cells that produce dopamine, a chemical messenger that enables normal body movements. Essential tremor also appears to involve a disruption in the activity of motor pathways, but it’s uncertain whether there is any loss of brain cells. Postmortem examinations of brain tissue taken from people with essential tremor reveal various abnormalities in the cerebellum and brainstem, including the loss of Purkinje cells, which produce an important neurotransmitter, called GABA. But the samples don’t all show the same changes, so the import of these findings is uncertain.

What Is A Parkinsons Tremor

Other health issues can also cause tremors, like multiple sclerosis or essential tremor. But Parkinsonâs tremors are different because theyâre usually:

  • Resting. Parkinsonâs tremors happen when your muscles are still. They go away when you move. They also lessen while you sleep. For example, if youâre sitting in a chair with your arm relaxed, you may notice that your fingers twitch. But if youâre using your hand, like when you shake someone elseâs hand, the tremor eases or stops.
  • Rhythmic. Parkinsonâs tremors are slow and continuous. They arenât random tics, jerks, or spasms.
  • Asymmetric. They tend to start on one side of your body. But they can spread to both sides of the body.

Diagnosing Parkinsons Vs Essential Tremor

The difference between essential tremor and Parkinsons can be so slight that it can be difficult for doctors to diagnose these conditions correctly. In fact, they are often mistaken for one another, which can be frustrating for physicians and patients alike. Specialty physicians that diagnose and treat these conditions are Movement Disorder Neurologists.

While there are no official tests for either condition, there are several diagnostic techniques that physicians employ to attempt properly diagnose essential tremor vs. Parkinsons, including:

  • Physical examination: During a physical examination, a physician evaluates the patients motor skills to help in their determination of whether they suffer from essential tremor vs. Parkinsons.

They include essential tremor and dystonic tremor and may be associated with other conditions and genetic mutations, although a specific gene has not been identified.

A patient with a Parkinsons tremor typically will find that their symptoms worsen and spread to other parts of the body over time. Generally, the tremor starts in one hand and can progress to the arm and foot on the same side of the body, and then eventually to the other side of the body. In severe cases, tremors also can affect the jaw or lips.

What Lifestyle Changes Can I Make To Ease Parkinsons Symptoms

Exercise: Exercise helps improve muscle strength, balance, coordination, flexibility, and tremor. It is also strongly believed to improve memory, thinking and reduce the risk of falls and decrease anxiety and depression. One study in persons with Parkinsons disease showed that 2.5 hours of exercise per week resulted in improved ability to move and a slower decline in quality of life compared to those who didnt exercise or didnt start until later in the course of their disease. Some exercises to consider include strengthening or resistance training, stretching exercises or aerobics . All types of exercise are helpful.

Eat a healthy, balanced diet: This is not only good for your general health but can ease some of the non-movement related symptoms of Parkinsons, such as constipation. Eating foods high in fiber in particular can relieve constipation. The Mediterranean diet is one example of a healthy diet.

Preventing falls and maintaining balance: Falls are a frequent complication of Parkinson’s. While you can do many things to reduce your risk of falling, the two most important are: 1) to work with your doctor to ensure that your treatments whether medicines or deep brain stimulation are optimal and 2) to consult with a physical therapist who can assess your walking and balance. The physical therapist is the expert when it comes to recommending assistive devices or exercise to improve safety and preventing falls.

Resolving A Dissonant Diagnosis

Martinezs grandfather had Parkinsons disease, so she knew she had a family history. While both she and her primary care physician suspected she could also have the condition, specialists told her she was too young, and diagnosed the problem as essential tremor . When their treatments for essential tremor failed, her primary care physician referred her to Tao Xie, MD, PhD, director of the Parkinsons Disease and Movement Disorder Clinic.

Misdiagnosis of essential tremor is not uncommon, and many people come to me for a second opinion after theyve been treated elsewhere, Xie said. Quite often theyre very nervous initially, because they have seen so many other doctors and they dont know whats going on.

To confirm the diagnosis of Parkinsons disease, Martinez underwent DaTscan imaging, a molecular imaging method that helps physicians detect levels of the protein that transports dopamine to cells in the brain. This technique shows how much degeneration has occurred in the areas of the brain that produce and transmit dopamine. Generally available only at academic medical centers, the scan is typically used as needed after a brain MRI rules out certain conditions that mimic Parkinsons symptoms. Xie says symptoms of actual Parkinsons generally dont appear until patients have lost about half the nerve cells involved in dopamine production. DaTscan will show that loss. It will also rule out essential tremor, because the neurons are not lost with that diagnosis.

Dementia With Lewy Bodies

  • Dementia with Lewy bodies is a progressive, neurodegenerative disorder in which abnormal deposits of a protein called alpha-synuclein build up in multiple areas of the brain.
  • DLB first causes progressive problems with memory and fluctuations in thinking, as well as hallucinations. These symptoms are joined later in the course of the disease by parkinsonism with slowness, stiffness and other symptoms similar to PD.
  • While the same abnormal protein is found in the brains of those with PD, when individuals with PD develop memory and thinking problems it tends to occur later in the course of their disease.
  • There are no specific treatments for DLB. Treatment focuses on symptoms.

Deep Brain Stimulation A Surgical Approach

Deep brain stimulation is recommended for those patients who are at their late stage of the disease or in a situation where medical therapy has completely failed to control the tremor. It has now become a standard treatment approach and is carried out in almost every big medical centre.

It required a surgery during which a small chip containing electrodes is installed in the brain. The chip works by reawakening parts of the brain responsible for movement function. The electrodes of the chip are connected to a pacemaker-like device placed under the skin in the upper chest area. The device controls the amount of stimulation from electrodes in the brain.

This surgical procedure is quite safe and effective. But it does have some risks and caused some side effects like depression, anxiety, seizures, dizziness, allergic reactions, loss of balance, and memory disturbance. Also, the whole surgical process is not pleasant and some neurologists may hesitate performing it.

Still, this treatment approach is very rewarding. Those who had this surgical procedure are very pleased with its outcome.

What Causes Parkinsons Disease

Parkinsons disease occurs when nerve cells in an area of the brain called the substantia nigra become impaired or die. These cells normally produce dopamine, a chemical that helps the cells of the brain communicate . When these nerve cells become impaired or die, they produce less dopamine. Dopamine is especially important for the operation of another area of the brain called the basal ganglia. This area of the brain is responsible for organizing the brains commands for body movement. The loss of dopamine causes the movement symptoms seen in people with Parkinsons disease.

People with Parkinsons disease also lose another neurotransmitter called norepinephrine. This chemical is needed for proper functioning of the sympathetic nervous system. This system controls some of the bodys autonomic functions such as digestion, heart rate, blood pressure and breathing. Loss of norepinephrine causes some of the non-movement-related symptoms of Parkinsons disease.

Scientists arent sure what causes the neurons that produce these neurotransmitter chemicals to die.

While currently available Parkinsons treatments cannot slow or halt the disease, they can help manage symptoms. According to the American Parkinson Disease Association, here are some of the most common treatment options:

    What Medications Are Used To Treat Parkinsons Disease

    Medications are the main treatment method for patients with Parkinsons disease. Levodopa is usually taken with carbidopa to allow more levodopa to reach the brain and to prevent or reduce the nausea and vomiting, low blood pressure and other side effects of levodopa. Sinemet® is available in an immediate release formula and a long-acting, controlled release formula. Rytary® is a newer version of levodopa/carbidopa that is a longer-acting capsule. The newest addition is Inbrija®, which is inhaled levodopa. It is used by people already taking regular carbidopa/levodopa for when they have off episodes .
