Those with essential tremor tend to have more unsteady and wavy writing, whereas Parkinsons patients are more apt to display micrographia, or abnormally small handwriting.

Tremor In Other Conditions

While tremor is a common symptom of Parkinsons, it can also be a symptom of other conditions, most notably essential tremor. The main difference between Parkinsons tremor and most other types of tremor is that in Parkinsons resting tremor is most common. Other conditions are usually characterized by action tremor, which tends to lessen at rest and increase when youre doing something, like trying to make a phone call or take a drink.

Tremors of the head and voice are also common in essential tremor but rare in Parkinsons.

What Is The Physiological Tremor

Tremor is a common side effect of many drugs . Various drugs and toxins can cause all types of tremor known clinically although increased physiological tremor is most commonly seen. Tremor is the dose limiting side effect of the ?2 adrenergic agonists, salbutamol and terbutaline, used to treat obstructive airway diseases. Tremor is usually seen within a month of starting valproic acid treatment and is more evident when a dose is >750 mg/day although it can also occur when the dose is within therapeutic range. It is the most common tremorogenic drug among anticonvulsants, affecting up to 25% of patients. Intention tremor may occur in patients on lithium. The occurrence rate increases with increasing serum lithium levels and manifests almost 100% in patients with lithium toxicity. Tardive tremor, a rare disorder, represents a separate entity in which, by definition, is caused by exposure to a dopamine receptor blocking agent within six months of the onset of symptoms and persisting for at least one month after stopping the offending drug. It is usually static in nature but can occur at rest and on intentional movements, such as eating and writing. Tremor can also occur as a toxic reaction to marijuana, and 3,4-methylene-dioxymethamphetamine or ecstasy.

Essential tremor

Essential tremor is one of the most common movement disorders.  The exact cause of essential tremor is unknown.  For some people this tremor is mild and remains stable for many years.  The tremor usually appears on both sides of the body, but is often noticed more in the dominant hand because it is an action tremor.

The key feature of essential tremor is a tremor in both hands and arms, which is present during action and when standing still.  Additional symptoms may include head tremor without abnormal posturing of the head and a shaking or quivering sound to the voice if the tremor affects the voice box.  The action tremor in both hands in essential tremor can lead to problems with writing, drawing, drinking from a cup, or using tools or a computer.

Tremor frequency may decrease as the person ages, but the severity may increase, affecting the persons ability to perform certain tasks or activities of daily living.  Heightened emotion, stress, fever, physical exhaustion, or low blood sugar may trigger tremor and/or increase its severity.  Though the tremor can start at any age, it most often appears for the first time during adolescence or in middle age .  Small amounts of alcohol may help decrease essential tremor, but the mechanism behind this is unknown.

What Treatments Are There For Tremors

Parkinsons tremors cannot be cured. However, there are several options to manage them.

Many of the typical Parkinsons drug treatments are associated with a reduction in tremors, including and dopamine agonists. A surgical procedure called deep brain stimulation also may be offered to control unmanageable tremors.

Physical therapy can help some patients control their tremors better, as can reducing the intake of substances such as caffeine, which can induce tremors. Many patients experience an increase in the severity of their tremors when they are . Therefore, trying to reduce sources of anxiety and engaging in complementary therapies may help some patients.

How Is Tremor Diagnosed

Tremor is diagnosed based on a physical and neurological examination and an individuals medical history.  During the physical evaluation, a doctor will assess the tremor based on:

  • whether the tremor occurs when the muscles are at rest or in action
  • the location of the tremor on the body
  • the appearance of the tremor .

The doctor will also check other neurological findings such as impaired balance, speech abnormalities, or increased muscle stiffness.  Blood or urine tests can rule out metabolic causes such as thyroid malfunction and certain medications that can cause tremor.  These tests may also help to identify contributing causes such as drug interactions, chronic alcoholism, or other conditions or diseases.  Diagnostic imaging may help determine if the tremor is the result of damage in the brain.

Additional tests may be administered to determine functional limitations such as difficulty with handwriting or the ability to hold a fork or cup.  Individuals may be asked to perform a series of tasks or exercises such as placing a finger on the tip of their nose or drawing a spiral.

The doctor may order an electromyogram to diagnose muscle or nerve problems. This test measures involuntary muscle activity and muscle response to nerve stimulation.

What Is A Tremor And What Makes It Different With Parkinsons

The Right Diagnosis Can Save Time

Because the symptoms of Parkinsons vary and often overlap other conditions, it is misdiagnosed up to 30% of the time, Dr. Fernandez says. Misdiagnosis is even more common in the early stages.

Patients who dont know where to turn may make appointments with a rheumatologist, or an orthopaedic or heart specialist, and undergo MRIs, EMGs and other expensive tests.

But only a neurologist can distinguish Parkinsons from essential tremor, drug-induced Parkinsons and Parkinsons plus syndromes, he says.

If patients come to us with typical signs of Parkinsons, we dont need to order expensive tests, he says.

Instead, neurologists base their diagnosis on a detailed patient exam and medical history, along with other information from the patient, family members or caregivers.

The term essential is not meant to indicate the tremors value. The shaking is an annoyance and can even be debilitating in some cases. The word essential is intended to indicate that tremor is the only symptom involved that its the essence of the entire problem. Similarly, the word benign signifies that the disorder, while annoying, is not inherently dangerous.

These views, while comforting and widely held, might be wrong.

For one thing, essential tremor is probably not actually just one disease, but rather a symptom caused by any one of a number of different processes.?? Some of these may indeed be benign, but others may be more serious.

Tremors Not Always Caused By Parkinsons

While researching Parkinsons Awareness Month, I came across a question posted on Providence Health Services website that took me down a bunny trail. The patient asked, Is the shaking in my left hand, which was diagnosed as an essential tremor related to Parkinsons disease?

Dr. Kiren Kresa-Reahl, a neurologist with Providence, replied. Theres a common misconception that everything that shakes is Parkinsons disease, but thats not true. Both essential tremor and Parkinsons cause hand tremors, but they are completely different disorders. Having an essential tremor does not mean that you have or will get Parkinsons disease.

Following that line of thinking, not all people with Parkinsons have a tremor, either. So that led me to follow the rabbit and to start treading about tremors and what possibly could be the cause and certainly the effect.

Tremors are classified into two main categories, resting, and action. The resting tremor is most often seen in people with Parkinsons. It occurs when the muscle is relaxed. An action tremor occurs with the voluntary movement of a muscle.

The National Institute of Neurological Disorders and Stroke drills the description of action tremors down to more classifications, many of which overlap. Postural tremor occurs when a person maintains a position against gravity, such as hold the arms outstretched. Kinetic tremor is associated with any voluntary movement, such as moving the wrists up and down or closing and opening the eyes.

What Are The Primary Motor Symptoms Of Parkinsons Disease

There are four primary motor symptoms of Parkinsons disease: tremor, rigidity, bradykinesia and postural instability . Observing two or more of these symptoms is the main way that physicians diagnose Parkinsons.

It is important to know that not all of these symptoms must be present for a diagnosis of Parkinsons disease to be considered. In fact, younger people may only notice one or two of these motor symptoms, especially in the early stages of the disease. Not everyone with Parkinsons disease has a tremor, nor is a tremor proof of Parkinsons. If you suspect Parkinsons, see a neurologist or movement disorders specialist.

There are different types of tremors, of which not all are associated with Parkinsons disease, and they vary slightly in terms of how, when and where they manifest in the body. They include essential tremor and dystonic tremor and may be associated with other conditions and genetic mutations, although a specific gene has not been identified.

A patient with a Parkinsons tremor typically will find that their symptoms worsen and spread to other parts of the body over time. Generally, the tremor starts in one hand and can progress to the arm and foot on the same side of the body, and then eventually to the other side of the body. In severe cases, tremors also can affect the jaw or lips.

Which Body Parts Do Parkinsons Tremors Affect

There are five main places youll have Parkinsons tremors:

1. Hands. Parkinsons disease tremors often start in the fingers or hands with whats called a pill-rolling motion. Imagine holding a pill between your thumb and index finger and rolling it back and forth.

2. Foot. A Parkinsons foot tremor is more likely to happen while youre sitting or lying down with your feet at rest. If the tremor moves into your thigh muscles. It could look like your whole leg is shaking.

Foot tremors disappear when you stand or walk because those are active movements. A foot or leg tremor while youre standing may be another condition.

Chewing eases the tremor, so gum might help.

4. Tongue. Its rare, but a tremor can cause your entire head to shake.

Can Dogs Get Parkinsons Disease

In some ways, dogs and people are similar.

Both express love, have a playful side, and remain fiercely loyal to their families.

Unfortunately, just like people, dogs also get Parkinsons disease.

Parkinsons is a neurological condition that attacks the motor system and typically leads to mental incapacity in the form of dementia.

If your dog has it, its important to get a diagnosis as soon as possible so that you can slow down the progress of the disease.

Environmental Factors And Exposures

Exposure to and a history of head injury have each been linked with PD, but the risks are modest. Never having smoked cigarettes, and never drinking caffeinated beverages, are also associated with small increases in risk of developing PD.

Drug-induced parkinsonism

Different medical drugs have been implicated in cases of parkinsonism. Drug-induced parkinsonism is normally reversible by stopping the offending agent. Drugs include:

Can Et And Pd Occur Together

Even more frightening is when PD or other parkinsonism syndrome occurs along with ET. Parkinsonism refers to conditions that resemble PD in many ways but are not true PD. A big difference is that medications that work for PD, such as L-dopa, have little or no effect on parkinsonism. In addition, a person with parkinsonism may not have tremors but will have other similar PD symptoms: bradykinesia , stooped posture, stiff walking, small steps, and reduced arm swinging when walking. Treatment using Deep Brain Stimulation does not help with parkinsonism.

There are other variants that are considered atypical Parkinsons disease . There is also a very rare condition called Progressive Supranuclear Palsy . In its early stages it also looks like PD or one of the forms of parkinsonism but with ongoing degeneration, stiffness, dizziness and falls become worse. There can also be personality and cognitive changes. Since any of the above can co-occur with ET, gaining an accurate diagnosis of co-existing conditions can be a confusing process. Symptoms overlap, so trying to tease out one diagnosis from another is challenging.

Recommendation For Those With Et

ET, PD and parkinsonism have one thing in common: they dont affect individuals in identical ways. There are accepted diagnostic guidelines, but an infinite number of small or large differences can occur: rate of progression, response to medication, nature of lifestyle problems and impairment, etc. If two persons with ET were to compare notes, they would quickly find that things like propranolol helps one but not the other, or one of them gets terrible restless leg syndrome at night while the other has never had it, etc. Thus, there is a growing theory that ET is not a single disorder, but a group of disorders.

For those with ET who notice that they have additional symptoms that dont fit the classic ET descriptions in books or websites, we recommend discussing the situation with a movement disorder specialist. Perhaps theres no cause for concern, but it may be worth seeking an evaluation to rule out a co-occurring form of parkinsonism. Accurate information may be your best friend, since it both equips you and your doctor to strategize treatment, and also to manage anxiety and gain peace of mind.
