Pharmacology – DRUGS FOR PARKINSON’S DISEASE (MADE EASY)

Deep brain stimulation is a surgical intervention used to treat movement disorders such as Parkinsons disease when the regimen of existing medications and the various rehabilitation strategies become less effective in managing symptoms. This surgical procedure received approval from the Food and Drug Administration to treat tremor in Parkinsons disease in July 1997 and for advanced motor symptoms of Parkinsons disease in January 2002.

For people with Parkinsons disease, deep brain stimulation surgery may be helpful if the individual is experiencing motor fluctuations including dyskinesias or off episodes.

The subthalamic nucleus and the globus pallidus are two locations in the brain that are targeted in the DBS procedure for Parkinsons. These include tremor, rigidity, slowness of movement, difficulty walking, and freezing, as well as extra movements or dyskinesia that are medication side effects. Inclusion And Exclusion Criteria

How To Take Entacapone

  • Before you start the treatment, read the manufacturer’s printed information leaflet from inside the pack and any additional information your doctor has given to you. These will give you more information about entacapone, and will also provide you with a full list of side-effects which you could experience from taking it.
  • Take the tablets exactly as your doctor tells you to. If you are prescribed Comtess®, take one 200 mg tablet at the same time as your co-careldopa or co-beneldopa tablet or capsule. If you are prescribed Stanek®, Stalevo® or Sastravi® tablets, these are available in different strengths. You will be prescribed the strength of tablet that best suits your condition. Your doctor will tell you how to take the tablets, and the directions will also be on the label of the pack to remind you about what the doctor said to you.
  • You can take entacapone either before or after meals.
  • When you first start taking entacapone, your doctor may need to adjust the doses of your other medicines. This is to help reduce any side-effects caused by these tablets increasing the effectiveness of your usual dose of levodopa.

The main reagent, entacapone, is a product of the Orion Corporation. Indomethacin, TTX, bumetanide, and DPC were purchased from Sigma . Preliminary experiments showed that the solvent did not alter basic electrophysiologic parameters. What Should I Watch For While Using This Medicine

Visit your doctor or health care professional for regular checks on your progress. Do not take any additional medicines for Parkinson’s disease without first consulting with your health care provider.

Entacapone may increase the side effects caused by Levodopa Carbidopa such as nausea or restless movements. If you notice an increase in or the appearance of certain side effects that occurred only while you are taking Levodopa Carbidopa, contact your physician. The dose of Levodopa-Carbidopa may need to be lowered. Do not decrease your medicine dose without asking your doctor or health care professional.

You may get dizzy or have difficulty controlling your movements. Do not drive, use machinery, or do anything that needs mental alertness until you know how this medicine affects you. Do not stand or sit up quickly, especially if you are an older patient. This reduces the risk of dizzy or fainting spells. Alcohol can increase possible dizziness. Avoid alcoholic drinks.

If you find that you have sudden feelings of wanting to sleep during normal activities, like cooking, watching television, or while driving or riding in a car, you should contact your health care professional.

Your mouth may get dry. Chewing sugarless gum or sucking hard candy, and drinking plenty of water may help. Contact your doctor if the problem does not go away or is severe.

Are There Any Other Precautions Or Warnings For This Medication

Before you begin taking a medication, be sure to inform your doctor of any medical conditions or allergies you may have, any medications you are taking, whether you are pregnant or breast-feeding, and any other significant facts about your health. These factors may affect how you should take this medication.

Behavioural and mood changes: This medication may cause mood and psychiatric changes, including hallucinations, hypersexuality, pathological gambling, and increased libido. Tell your doctor as soon as possible if you notice any unusual changes in your behaviour, have thoughts of harming yourself, or feel depressed.

If you have depression or a history of depression, discuss with your doctor how this medication may affect your medical condition, how your medical condition may affect the dosing and effectiveness of this medication, and whether any special monitoring is needed. If you experience symptoms of depression such as poor concentration, changes in weight, changes in sleep, decreased interest in activities, or notice them in a family member who is taking this medication, contact your doctor as soon as possible.

Diarrhea: Diarrhea is a common side effect of this medication. It may appear as early as the first week of starting treatment or many months after starting treatment. For some people, the diarrhea causes weight loss. If you notice any weight loss or have excessive diarrhea, contact your doctor.

In some cases, your doctor may prescribe other drugs for you to take with Stalevo. Additional drugs may help with motor symptoms of Parkinsons disease such as muscle rigidity, shaking, and dyskinesia .

Symptomatic Treatment Of Parkinson Disease

Many types of medications are available for symptomatic treatment of PD: anticholinergics, amantadine, LD, MAO inhibitors , catechol-O-methyltransferase inhibitors , and DAs . Anticholinergics such as trihexyphenidyl and benztropine antagonize the effects of acetylcholine at muscarinic receptors postsynaptic to striatal interneurons. They reduce tremor and rigidity but have no effects on bradykinesia. Toxicity relates to antagonism of acetylcholine at central receptors, causing confusion, and peripheral receptors, causing blurred vision, dry mouth, constipation, and urine retention. Although amantadine has been available for nearly 4 decades , its antiparkinsonian mechanisms have been poorly understood. It has been thought to stimulate release of endogenous dopamine stores, block reuptake of dopamine from the synaptic cleft, and have anticholinergic properties. However, amantadine has been found to have antiglutamatergic properties and as such is the only antiparkinsonian drug that improves LD-induced dyskinesia. Extended release formulation of amantadine has been found to improve not only dyskinesia but also motor fluctuations .

Some Disadvantages Of Comt Inhibitors

These drugs can increase the side effects caused by levodopa, notably dyskinesias , nausea and vomiting.

If these side effects increase after starting the drug, people should raise the issue with their healthcare professional, as reducing the levodopa dose can often help.

COMT inhibtors will discolour urine making it a reddish-brown colour. Some people also experience diarrhoea which may occur some months after commencing the medication.

Be aware that other drugs for Parkinsons or other conditions can affect the action of COMT inhibitors. The combination of apomorphine and entacapone needs careful supervision.

Stalevo And Herbs And Supplements

Stalevo may interact with supplements that contain iron. This is because supplements with iron may decrease the amount of levodopa your body absorbs, and Stalevo contains levodopa.

Therefore, taking your dose of Stalevo with a multivitamin or supplement that contains iron may cause your dose of Stalevo to be less effective.

If you need to take a multivitamin or supplement that contains iron while youre taking Stalevo, talk with your doctor about the best time to take each treatment.

Recording Of Rat Colon Smooth Muscle Contraction

The rats were killed and then distal colon was quickly removed. The luminal contents of the colon were washed with Krebs solution. The 1st to 4th segments of the distal colon were collected. Each segment was cut open along the mesentery border and spread evenly over silica gel with the apical membrane facing down. The samples were surrounded by ice-cold Krebs solution to maintain tissue activity. The entire colon was cut into 2-mm-wide, 1-1.5-cm-long strips along the longitudinal or circular muscles. Surgical sutures were used to tie a piece of thread to each end of the muscle strip and connect one end to a specimen support rod and the other end to a pressure transducer. The muscle strips were incubated in 10 mL of Krebs solution at a constant temperature under continuous gas ventilation . After applying 1g of resting muscle tension, the change in length was recorded by the pressure transducer. With 2000 mg of weight for calibration, intensity of muscle strip contraction was measured with mg as the basic unit. After the muscle strips were adapted in a water bath for 120 min, the autonomous contraction intensity was officially recorded.

What Other Information Should I Know

Keep all appointments with your doctor and the laboratory. You may become dizzy when you get up after sitting or lying down, especially when you begin taking entacapone. To avoid this problem, make sure to get up slowly, especially if you have been sitting or lying down for a long time.

Entacapone may cause your urine to change to a brownish-orange color. This effect is common and is not harmful.

Comtan In Clinical Trials

Comtan was approved by the U.S. Food and Drug Administration in October 1999. The safety and efficacy of the drug were confirmed through three randomized, double-blind, placebo-controlled trials, which led to its approval.

In two studies , Comtan was specifically assessed for its ability to reduce the level of motor fluctuations . Over 24 weeks, patients who had documented levodopa-induced motor fluctuations were prescribed either 200 mg of Comtan or a placebo alongside levodopa treatment.

Both studies showed a significant increase in the amount of on hours in the Comtan group compared to the placebo group. In the Nordic group there was an increase of 1.5 hours , and an increase of 1 hour in the North American group. There was also a significant improvement in Unified Parkinsons Disease Rating Scale scores, which assesses the change in the general movement ability of the patients and the ability to perform activities of daily life such as dressing and walking.

A third study, in Germany and Austria, assessed a range of patients with or without motor fluctuations. A similar significant and positive impact on UPDRS scores was observed compared to the placebo.
