There is no cure for Parkinsons but genetic testing can be done to assess your risk for early diagnosis. In conventional medicine, Parkinsons treatment is focused on slowing down the progression of the disease through medications that either imitate dopamine in the brain, convert to dopamine in the brain, or slow down the breakdown of dopamine.

The problem with these medications is that they often have their own set of side effects that can be equally as debilitating to live with as Parkinsons symptoms with hallucinations being the one of them. But the question is, can you live with Parkinsons without medication? If so, can Parkinsons disease be reversed naturally?

In functional medicine we aim to both slow down progression and put symptoms into remission without the need for medication. While sometimes medication is necessary, we look for solutions that offer the least amount of side effects that are still effective at addressing symptoms. Sometimes a combination of medication and natural solutions are needed depending on how far along Parkinsons has progressed and sometimes alternative solutions can be enough to be effective.

An Approach To The Treatment Of Parkinson’s Disease

No treatment can arrest or slow neurodegeneration in Parkinson’s disease. The aim is to relieve symptoms and avoid the complications of therapy.

Early Parkinson’s disease

Many studies have shown that early treatment with dopamine agonists reduces the incidence of dyskinesia.1Fewer motor fluctuations were shown in some but not all of the studies. We recommend a dopamine agonist as the first treatment in younger patients who have mild disease and no cognitive deficit. It is necessary to add levodopa within 1-5 years in most patients. In more severe disease, treatment begins with levodopa but a dopamine agonist may be added to keep the daily dose of levodopa in the lower range if there is no cognitive deficit. Dopamine agonists are used infrequently and with caution in patients more than 70 years old because of the risk of neuropsychiatric adverse effects and postural hypotension. They are contraindicated in the presence of dementia.

Isolated resting tremor is rarely disabling, but if it interferes with function it can usually be managed with levodopa. When this is ineffective at low to moderate doses, the addition of an anticholinergic can sometimes be useful.

Changes In Cognition And Parkinsons Disease

Some people with Parkinsons may experience changes in their cognitive function, including problems with memory, attention, and the ability to plan and accomplish tasks. Stress, depression, and some medications may also contribute to these changes in cognition.

Over time, as the disease progresses, some people may develop dementia and be diagnosed with Parkinsons dementia, a type of Lewy body dementia. People with Parkinsons dementia may have severe memory and thinking problems that affect daily living.

Postural hypotension

Levodopa and dopamine agonists worsen postural hypotension and it may be necessary to lower the dose of levodopa or withdraw the agonist. Treatment is difficult, but patients should be advised to sleep with the head of the bed raised by one or two bricks and to add salt to their diet. Fludrocortisone can then be added at a dose of 0.1 mg in the morning, increasing if necessary up to 0.5 mg in the morning. If these measures are ineffective, the alpha agonist midodrine 10-20 mg four hourly can be useful but it is experimental and only available via the Special Access Scheme. Patients treated for postural hypotension need to have electrolytes, renal function and supine blood pressures closely monitored.

Impulsive And Compulsive Behaviours

People who experience impulsive and compulsive behaviours cant resist the temptation to carry out an activity often one that gives immediate reward or pleasure.

Behaviours may involve gambling, becoming a shopaholic, binge eating or focusing on sexual feelings and thoughts. This can have a huge impact on peoples lives including family and friends.

Not everyone who takes Parkinsons medication will experience impulsive and compulsive behaviours, so these side effects should not put you off taking your medication to control your symptoms.

If you have a history of behaving impulsively you should mention this to your GP, specialist or Parkinsons nurse.

Asking your specialist to make changes to your medication regime or adjusting the doses that you take is the easiest way to control impulsive and compulsive behaviours. Common Drugs For Parkinson’s Disease

Levodopa and carbidopa . Levodopa is the most commonly prescribed medicine for Parkinsonâs. Itâs also the best at controlling the symptoms of the condition, particularly slow movements and stiff, rigid body parts.

Levodopa works when your brain cells change it into dopamine. Thatâs a chemical the brain uses to send signals that help you move your body. People with Parkinsonâs donât have enough dopamine in their brains to control their movements.

Sinemet is a mix of levodopa and another drug called carbidopa. Carbidopa makes the levodopa work better, so you can take less of it. That prevents many common side effects of levodopa, such as nausea, vomiting, and irregular heart rhythms.

Sinemet has the fewest short-term side effects, compared with other Parkinsonâs medications. But it does raise your odds for some long-term problems, such as involuntary movements. An inhalable powder form of levodopa and the tablet istradefylline have been approved for those experiencing OFF periods, OFF periods can happen when Parkinsonâs symptoms return during periods between scheduled doses of levodopa/carbidopa.

People who take levodopa for 3-5 years may eventually have restlessness, confusion, or unusual movements within a few hours of taking the medicine. Changes in the amount or timing of your dose will usually prevent these side effects.

Dopamine agonists. These drugs act like dopamine in the brain. They include pramipexole , rotigotine , and ropinirole .

Speech And Occupational Therapy

Parkinsons disease can lead to slurred speech and difficulty swallowing. A speech and language therapist can provide muscle training techniques that may help overcome some of these problems.

An occupational therapist can help identify everyday tasks that can be challenging and work with the person to find practical solutions.

This may include new strategies for dressing, preparing meals, performing household chores, and shopping. Adaptations to the home environment can also make daily living easier.

For people with Parkinsons disease, deep brain stimulation may help manage:

  • an electrode inside the part of the brain that controls movement
  • a pacemaker-like device, or neurostimulator, under the skin in the upper chest
  • a wire under the skin connecting the neurostimulator to the electrode

The neurostimulator sends electrical impulses along the wire and into the brain via the electrode. These impulses can prevent symptoms by interfering with the electrical signals that cause them.

There is a small risk of brain hemorrhage, infection, and headaches. Some people may see no improvement, or their symptoms may worsen. There may also be discomfort during stimulation.

Nevertheless, the AAN considers this treatment safe and effective for specific people and say any adverse effects are usually mild and reversible. Anyone considering this treatment should discuss the pros and cons with a healthcare professional.

Levodopa Or Dopamine Agonists: What Does Research Say

Several studies have looked into the long-term effects of the initial levodopa vs. dopamine agonist treatment decision. For instance, one large study showed the following after seven years:

  • About 50 out of 100 people had stopped their initial treatment with dopamine agonists because the side effects were too bad or the medication wasnt effective enough.
  • In comparison, at the same stage only 7 out of 100 people had stopped their initial treatment with levodopa.

A New Era For Parkinsons Disease Treatment

A non-invasive ultrasound treatment for Parkinsons disease that was tested in a pivotal trial led by University of Maryland School of Medicine researchers is now broadly available at the University of Maryland Medical Center .

Parkinson’s Disease Is More Common Than Thought Toxins May Contribute To Rising Rates

Parkinson’s disease strikes nearly 90,000 older Americans a year, 30,000 more than was previously estimated, according to a study published Thursday.

Incidence rates differed across the country. States with higher rates of older residents saw more diagnoses of the disease, whose risk typically increases with age, but so didsome “Rust Belt” states in the Northeast and Midwest that have a history of heavy industry manufacturing.

Increasing rates of Parkinson’s and the cost of treatments and lost income means everyone will be touched by the disease either directly or indirectly, said Dr. Michael Okun, chair of neurology at the University of Florida College of Medicine in Gainesville.

How Should This Medicine Be Used

The combination of levodopa and carbidopa comes as a regular tablet, an orally disintegrating tablet, an extended-release tablet, and an extended-release capsule to take by mouth. The combination of levodopa and carbidopa also comes as a suspension to be given into your stomach through a PEG-J tube or sometimes through a naso-jejunal tube using a special infusion pump. The regular and orally disintegrating tablets are usually taken three or four times a day. The extended-release tablet is usually taken two to four times a day. The extended-release capsule is usually taken three to five times a day. The suspension is usually given as a morning dose and then as a continuous dose , with extra doses given no more than once every 2 hours as needed to control your symptoms. Take levodopa and carbidopa at around the same times every day. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand. Take levodopa and carbidopa exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Swallow the extended-release tablets whole do not chew or crush them.

To take the orally disintegrating tablet, remove the tablet from the bottle using dry hands and immediately place it in your mouth. The tablet will quickly dissolve and can be swallowed with saliva. No water is needed to swallow disintegrating tablets.
