How Does It Work
In people with Parkinsons disease, the brain doesnt produce enough of a neurotransmitter called dopamine. The cells that produce dopamine either die or become impaired. Dopamine is necessary for proper motor control and movement.
Specifically, dopamine transmits signals in the brain that are involved in smooth, purposeful movements like eating, writing, and typing. Like selegeline and rasagaline, safinamide is a type of MAO-B inhibitor, which prevents the breakdown of dopamine and thus increases its levels in the brain.
Of note, safinamide also modulates glutamate release however, the specific effect of this action on the drugs therapeutic actions is unknown.
Unlike other MAO-B inhibitors, which can be prescribed alone for those with early-stage Parkinsons disease, safinamide is intended to be used in conjunction with other types of antiparkinson drugs for the later-stage disease, most notably levodopa as well as dopamine agonists.
When people first start treatment for Parkinsons symptoms, drugs tend to work pretty well and symptoms are controlled throughout the day. Between five and 10 years, however, the efficacy of conventional Parkinsons drugs wanes in many people, and symptom control becomes more difficult to alleviate.
Specifically, in people with mid- to late-stage Parkinsons disease, motor fluctuations or involuntary muscle movements begin to crop up.

Generic Vs Branded Drugs
Currently, there are multiple pharmaceutical companies that manufacture a generic formulation of carbidopa-levodopa, dopamine agonists, monoamine oxidase inhibitors, and anticholinergics. The U.S. Food and Drug Administration requires that generic drugs show a similar risk and benefit to the branded drug prior to market approval, but in rare cases this standard is not high enough.
A review supported by the Parkinsons Foundation reports evidence that if you are in more advanced stages of the disease, switching from branded drugs to generic, or from one generic to another, may have somewhat variable effects. The authors, including Parkinsons Foundation National Medical Advisor Michael S. Okun, MD, believe that the standards for approving generic drugs for PD may not be strict enough to demonstrate that the generic alternatives are equally effective.
Work with your doctor to develop a tailored treatment plan. Using generic drugs will likely provide a cost savings. Infrequently, a person living with PD may require brand medication.
If you make the switch, follow these tips:
- Report to your physician on the effectiveness of the drugs.
- Carefully keep a diary of any side effects.
- Record dose adjustments made by your physicians .
- Try to stay with a single drug manufacturer for your generic medications. You may need to ask your pharmacist to special order for you.

Glucocerbrosidase Enhancing The Cells Lysosomal System
GBAis a gene that increases the risk of developing PD. The GBA protein works in the lysosome, the garbage disposal system of the cell, breaking down cellular products that can be harmful to the cell. Having two abnormal GBA genes causes Gauchers disease, which is characterized by the buildup of these cellular products. This results in fatigue, bone pain, easy bleeding and an enlarged spleen and liver. When a person inherits only one abnormal gene, he or she does not develop Gauchers disease however, they do incur a small increased risk of PD. Most people with one mutated GBA gene do not develop PD.
Enzyme replacement therapy, in which the GBA protein is given intravenously, is available as a treatment for Gauchers disease. This protein is too big to cross the blood-brain-barrier however, and so it does not enter the brain and does not treat any symptoms caused by the abnormal buildup of cellular components in the brain. The following strategies were developed in an attempt to compensate for the effects of the GBA mutation in the brain:
- Ambroxol, approved in Europe for respiratory illnesses, improves the function of GBA in neurons NCT02941822 and NCT02914366

Diet Plan Guidelines For Parkinsons Disease
- Fresh, whole food, unrefined, low-GI, diet We commonly start patients on an autoimmune paleo diet to begin with.
- Emphasise anti-oxidant and anti-inflammatory diet: mostly plant-based diet high in fresh fruits and vegetables, turmeric, berries, ginger, green tea etc. Meats to be organic or grass fed.
- Reduce intake of pro-inflammatory and pro-oxidant foods: excessive animal foods, damaged fats, sugars, refined carbohydrates and processed foods.
- Reduce or eliminate dairy foods: dairy product consumption is positively associated with Parkinsons disease, especially in men
- Increase foods that support nerve structure and function: egg yolks, oily fish, nuts and seeds etc
- Ensure adequate protein to support dopamine production : fish, eggs, grass fed meats, liver

Fda Approves New Indication For Adamas Gocovri In Parkinsons Disease
Gocovri approved to treat patients with Parkinsons disease experiencing OFF episodes
The US Food and Drug Administration has approved Adamas Pharmaceuticals Gocovri for a new indication in patients with Parkinsons disease experiencing OFF episodes.
The FDA has approved Gocovri as an adjunctive treatment to levodopa/carbidopa, a commonly-used treatment for the main symptoms of Parkinsons disease.
When Parkinsons progresses, individuals receiving levodopa-based therapy often experience the return of symptoms including stiffness, rigidity and tremors referred to as OFF episodes between medication doses.
Although the primary treatment for Parkinsons is levodopa, over time treatment with this drug can cause involuntary and uncontrolled movements referred to as dyskinesia.
In addition to this new indication approval for use in treating OFF episodes, Gocovri is approved to treat dyskinesia in Parkinsons patients receiving levodopa/carbidopa therapy.
In two phase 3 clinical studies, Gocovri treatment was shown to significantly reduce both OFF time and dyskinesia.
Gocovri treatment also resulted in a clinically meaningful increase in good ON time referring to when levodopa is working well and symptoms are controlled.
In addition, Adamas drug demonstrated sustained efficacy for at least two years in the phase 3 EASE LID-2 trial.
Gocovri is now the first and only medication approved to treat both OFF and dyskinesia motor complications in Parkinsons disease, he added.

Throughout my many years in Practice, I have had the opportunity to work with many patients suffering form neurodegenerative diseases, including those suffering from Parkinsons and this is what I have discovered. The human brain is particularly vulnerable to oxidative stress due to its high metabolic activity. Glucose is the main fuel of the brain, but when glucose is low , the brain can use lactate during exercise and ketone bodies for fuel during glucose depletion. The general reliance on glucose means that the brain uses disproportionately more oxygen than other tissues.
What does this have to do with Parkinsons? Well, when dopamine is made in the brain it is especially prone to oxidation. This is where high dose antioxidant therapy becomes extremely important and beneficial for those suffering from Parkinsons disease. Apart from the conventional application in advanced PD, DBS has also been suggested to exert disease-modifying traits . In multiple preclinical studies on rat models, chronic STN electrical stimulation was shown to result in preservation of SNpc dopaminergic neurons and an increase of brain-derived neurotrophic factors followed by activation of the tropomyosin receptor kinase type B receptor signaling in the nigrostriatal system .
