What is Parkinson’s Disease?

In 2011, the FDA approved the use of a scan called a dopamine transporter scan . A DaTscan is an imaging technology that allows visualization of the dopamine system in the brain. It is similar to an MRI, but looks at the function of the brain rather than the structure.

A DaTscan involves injection of a small amount of a radioactive drug that is then measured by a single-photon emission computed tomography scanner . The SPECT scanner measures the levels and location of the drug in the brain.

It is important to know that a negative DaTscan does not rule out PD, especially early in the disease, but a positive DaTscan can help confirm it. A positive DaTscan can differentiate PD from essential tremor as there is no dopamine deficiency in the latter. However, DaTscan abnormalities can be seen in PD as well as other forms of atypical parkinsonism that cause a loss of dopamine . This means that a positive result does not differentiate Parkinsons disease from other forms of atypical parkinsonism.

Referral To A Specialist

If your GP suspects Parkinson’s disease, you’ll be referred to a specialist.

This will usually be:

  • a neurologist, a specialist in conditions affecting the brain and nervous system
  • a geriatrician, a specialist in problems affecting elderly people

The specialist will most likely ask you to perform a number of physical exercises so they can assess whether you have any problems with movement.

A diagnosis of Parkinson’s disease is likely if you have at least 2 of the 3 following symptoms:

  • shaking or tremor in a part of your body that usually only occurs at rest
  • slowness of movement
  • muscle stiffness

If your symptoms improve after taking a medication called levodopa, it’s more likely you have Parkinson’s disease.

Special brain scans, such as a single photon emission computed tomography scan, may also be carried out in some cases to try to rule out other causes of your symptoms.

People who gain or lose weight soon after being diagnosed with Parkinson’s disease may be more likely to have changes in their thinking skills than people who maintain their weight, according to a study published in the October 19, 2022, online issue of Neurology┬«, the medical journal of the American Academy of Neurology.

“Early weight loss is a common symptom in people with Parkinson’s disease,” said study author Jin-Sun Jun, MD, of Kangnam Sacred Heart Hospital in Seoul, Republic of Korea. “It could serve as a sign that people are at risk of cognitive decline.”

The study involved 358 people who were recently diagnosed with Parkinson’s disease and had not yet started taking Parkinson’s medications. They were an average age of 61 and had been diagnosed an average of two years earlier. They were compared to 174 people who did not have Parkinson’s disease.

Weight gain or loss was defined as a change of more than 3% of body weight during the first year of the study. Weight maintenance was defined as no change or change of no more than 3%. A total of 98 people had weight loss, 59 had weight gain and 201 maintained their weight.

There was no association between weight change and any other non-motor symptoms.

The most prominent signs and symptoms of Parkinsons disease occur when nerve cells in the basal ganglia, an area of the brain that controls movement, become impaired and/or die. Normally, these nerve cells, or neurons, produce an important brain chemical known as dopamine. When the neurons die or become impaired, they produce less dopamine, which causes the movement problems associated with the disease. Scientists still do not know what causes the neurons to die.

People with Parkinsons disease also lose the nerve endings that produce norepinephrine, the main chemical messenger of the sympathetic nervous system, which controls many functions of the body, such as heart rate and blood pressure. The loss of norepinephrine might help explain some of the non-movement features of Parkinsons, such as fatigue, irregular blood pressure, decreased movement of food through the digestive tract, and sudden drop in blood pressure when a person stands up from a sitting or lying position.

Many brain cells of people with Parkinsons disease contain Lewy bodies, unusual clumps of the protein alpha-synuclein. Scientists are trying to better understand the normal and abnormal functions of alpha-synuclein and its relationship to genetic mutations that impact Parkinsons andLewy body dementia.

Whats Different About Young

The age of diagnosis matters for a variety of reasons, from probable causes of early cases to symptoms and treatment:

  • Genetics. As with any case of Parkinsons disease, the exact cause is usually unknown. That said, The young-onset cases of Parkinsons disease are, on average, a bit more likely to be familial or genetic, says Gregory Pontone, M.D., director of the Johns Hopkins Movement Disorders Psychiatry Clinic.
  • Symptoms. In many patients with YOPD, dystonia is an early symptom. People with YOPD also report more dyskinesia . They also tend to exhibit cognitive problems, such as dementia and memory issues, less frequently.
  • Progression. Patients with young-onset Parkinsons appear to have a slower progression of the disease over time, says Pontone. They tend to have a milder course, staying functional and cognitively intact for much longer.
  • Treatment. Most patients with Parkinsons take the medication levodopa. However, other drugs, such as MAO-B inhibitors, anticholinergics, amantadine, and dopamine receptor agonists, may be used before levodopa.

Physical Examination And Tests

There currently are no diagnostic blood tests for Parkinson’s disease, but your healthcare provider may do some routine blood and urine tests to assess your overall health. Your blood pressure will be taken sitting and standing to look for orthostatic hypotension.

Medications For People With Parkinsons Disease

Symptoms of Parkinsons disease result from the progressive degeneration of nerve cells in the brain and other organs such as the gut, which produce a neurotransmitter called dopamine. This causes a deficiency in the availability of dopamine, which is necessary for smooth and controlled movements. Medication therapy focuses on maximising the availability of dopamine in the brain. Medication regimes are individually tailored to your specific need. Parkinsons medications fit into one of the following broad categories:

  • levodopa dopamine replacement therapy
  • dopamine agonists mimic the action of dopamine
  • COMT inhibitors used along with levodopa. This medication blocks an enzyme known as COMT to prevent levodopa breaking down in the intestine, allowing more of it to reach the brain
  • anticholinergics block the effect of another brain chemical to rebalance its levels with dopamine
  • amantadine has anticholinergic properties and improves dopamine transmission
  • MAO type B inhibitors prevent the metabolism of dopamine within the brain.

Looking For Signs Of Parkinsons

How Is Parkinsons Treated

Although theres currently no cure for Parkinsons, treatments are available to help reduce symptoms and maintain quality of life for as long as possible. These include:

  • supportive therapies, such as physiotherapy and occupational therapy
  • deep brain stimulation .

If you have mild symptoms of Parkinsons, your doctor might not recommend medication and instead focus on supportive therapy and lifestyle improvements such as exercise and relaxation. As your symptoms progress, you will be prescribed medication.

How Is Parkinson’s Disease Diagnosed

Your doctor will ask questions about your symptoms and your past health and will do a neurological exam. This exam includes questions and tests that show how well your nerves are working. For example, your doctor will watch how you move. He or she will check your muscle strength and reflexes and will check your vision.

Your doctor also may check your sense of smell and ask you questions about your mood.

In some cases, your doctor will have you try a medicine for Parkinson’s disease. If that medicine helps your symptoms, it may help the doctor find out if you have the disease.

Tests

There are no lab or blood tests that can help your doctor know whether you have Parkinson’s. But you may have tests to help your doctor rule out other diseases that could be causing your symptoms. For example:

  • An MRI or CT scan is used to look for signs of a stroke or brain tumor.
  • Blood tests check for abnormal thyroid hormone levels or liver damage.

Another type of imaging test, called PET, sometimes may detect low levels of dopamine in the brain. These low levels are a key feature of Parkinson’s. But PET scanning isn’t commonly used to evaluate Parkinson’s. That’s because it’s very expensive, not available in many hospitals, and only used experimentally.

Molecular Imaging In Parkinson’s Disease

The diagnosis of PD relies on the clinical manifestation of cardinal motor symptoms, bradykinesia, and tremor at rest or rigidity . A positive response to dopaminergic drugs is supportive of the diagnosis. Single photon emission computed tomography or PET ligands that are specific for dopamine transporters indirectly enable the quantification of the deficit of dopaminergic nigrostriatal projections and can provide further support of diagnosis . Deficiencies of monoamine synthesis can be measured with dihydroxyphenylalanine which is a substrate for the enzyme aromatic amino acid decarboxylase in all monoaminergic neurons including noradrenergic neurons .

The role of deficits of noradrenaline in motor and non-motor symptoms is not clear and research on the noradrenergic system in PD patients has been hindered by lack of specific methods to visualize the noradrenergic neurons and projections in vivo. We have recently carried out PET studies to investigate the role of noradrenaline in non-motor symptoms in PD patients and these studies will form the basis of discussions in the paragraphs below.
