We are still learning about constipation in PD and why it happens. Here is what we know:

Parkinsons Disease

The same changes that occur in brain cells in Parkinsons disease may also occur in nerve cells in the spinal cord and the intestinal wall. These changes may slow down the muscles that push food through the intestines.

Medications

Medications used to treat PD in particular, the class called anticholinergics and the medication amantadine, used to treat dyskinesia are known for causing constipation. If you are on these medications, your healthcare provider may be able to reduce your dose or switch you to a different one. But for some people, the benefits of the medication outweigh the possibility of constipation.

Because people with Parkinsons disease experience difficulty with their movement, they often become less active. People with PD who increase their movement experience better overall functioning, which includes their digestive system.

Many people with Parkinsons disease limit their fluids to avoid making frequent trips to the bathroom. When a person drinks less liquid, the gut may not have the lubrication it needs to have a bowel movement, which contributes to constipation.

Genetic Predisposition

It is possible to have a family predisposition to constipation. Ask family members what solutions work for them. Your body may respond to the same strategies.

As Parkinsons disease can cause constipation, the Parkinsons Foundation recommends a diet featuring 20 to 25 grams of daily fiber to maintain bowel health.

Its really important for overall health to keep bowels moving, Subramanian says. We recommend a diet with a lot of vegetables and as much fiber as you can take. Foods that are high in prebiotics, including fermented foods like sauerkraut and kimchee, can also help.

Some Parkinsons disease medications dont work as well when taken with fermented foods, however, so check with your doctor before incorporating them into your diet.

Proper hydration is also important for everyone, including people who have Parkinsons disease. Try to drink six to eight glasses of water a day and take your medications with a full glass of water, the Parkinsons Foundation notes. It may help your body break down the medication more efficiently.

Hydration helps with blood pressure and constipation, Subramanian notes. We recommend our Parkinsons patients drink 40 ounces of water a day. Thats just water, not coffee or tea or other drinks. This can also help improve digestion.

If drinking water leads to urinary urgency, try eating foods with a high water content like celery, butternut squash, grapefruit, strawberries, and watermelon instead.

In Part 2 of this 3-part blog series we discuss constipation related to Parkinsons disease and the gut-brain connection.

In patients with Parkinsons disease, the major cause of constipation is that the nervous system control of the gut slows down due to the effects of the disease, resulting in the slowing down of gut movement and leading to constipation.

Alpha-synuclein refers to the abnormal protein that is also seen in the brains of patients with PD. These same protein deposits have been observed in the gut prior to development of motor symptoms, up to 20 years prior to diagnosis. These abnormalities have also been found in the parasympathetic neurons of the vagus and sacral nerves that connect the brain to the visceral organs and are involved in the autonomic nervous system. The term parasympathetic refers to the side of the autonomic nervous system that allows the gut to move . Basically as the nervous system control of the gut slows, gut function slows and constipation results.

Interestingly, patients self-reports of constipation may not paint the full picture of the severity of constipation. One study found that objective measurements of colon transit time revealed higher rates of constipation than subjectively reported by patients. This is known as slow transit constipation.

Patients with PD should be aware of their bowel movements and remain vigilant to the development of constipation and should do the following to improve bowel regularity:
