Tremor Disorder or Parkinson’s?

Cell transplantation is regarded as a potential future PD treatment. There have been trials using autologous and non-autologous cells. Human embryonic stem cells and induced pluripotent stem cells are few of the cells that have been included in these transplantation studies. The Differences Between Essential Tremor And Parkinsons Disease

Essential tremor and Parkinsons disease are often misdiagnosed, in part, because the signs of the two conditions include tremor. However, there are key differences between the two movement disorders:

  • Tremor timing: Essential tremor typically occurs at a higher frequency when a patient attempts to use their hands. Tremor from Parkinsons disease, on the other hand, occurs when a patients hands are at rest.
  • Other symptoms: While essential tremor is not known to cause any other health conditions, those with Parkinsons disease often develop a stooped posture, slowed movement, and a shuffling gait.
  • Body parts affected:Essential tremor often only affects the hands, head, and voice. Patients with Parkinsons disease will develop tremors in their legs, chin, and other parts of their body.

Classification Of Motor Phenotypes For Pd Patients

Motor symptoms were assessed by the Unified Parkinsons Disease Rating Scale III. Motor phenotypes were identified based on the ratio of mean tremor score to the mean bradykinesia/rigid score . Patients with a ratio greater than 1.0, less than 0.80, and between 0.80 and 1.0 were classified into PD-TD, PD-PIGD, and mixed-types , respectively. The patients of mixed-type were excluded due to the mixed motor phenotype.

Example Of A Body Memory

An an example, he recalled that when he was a four-year-old boy, he once overheard the grownups talking, and his grandma getting so upset that she left the house, saying the words I am going to kill myself in the lake. Even if it seems like a small thing in a grownups ear, it meant hours of anxiety for his boyhood self, waiting to see if his grandmother ever came back from her walk. These fearful feelings were lingering in my clients body for years until we tapped into this, and I facilitated a new understanding of this situation, so this trapped feeling could be felt safely, and then dissipate.

We can say, in brief, that a memory with a feeling, which was not processed properly at the time, is a potential source for any symptom because the body somehow stores these together until it is safe to be felt. If it is a dramatic event, we call it a trauma, but, as in the above example, it can also be seemingly relatively small things to an adults sensibilities, so we more generally call it a body memory.

In the above example, this body memory gave the child, and now the man, a feeling that the world is dangerous and loved ones and protectors can disappear and die. Having several similar episodes causes tension in the body build up.

Essential tremor

Essential tremor is one of the most common movement disorders. The exact cause of essential tremor is unknown. For some people this tremor is mild and remains stable for many years. The tremor usually appears on both sides of the body, but is often noticed more in the dominant hand because it is an action tremor.

The key feature of essential tremor is a tremor in both hands and arms, which is present during action and when standing still. Additional symptoms may include head tremor without abnormal posturing of the head and a shaking or quivering sound to the voice if the tremor affects the voice box. The action tremor in both hands in essential tremor can lead to problems with writing, drawing, drinking from a cup, or using tools or a computer.

Tremor frequency may decrease as the person ages, but the severity may increase, affecting the persons ability to perform certain tasks or activities of daily living. Heightened emotion, stress, fever, physical exhaustion, or low blood sugar may trigger tremor and/or increase its severity. Though the tremor can start at any age, it most often appears for the first time during adolescence or in middle age . Small amounts of alcohol may help decrease essential tremor, but the mechanism behind this is unknown.

What Makes A Parkinsons Tremor Different

The tremor that occurs in Parkinsons disease is different from almost all other tremors because it is a resting tremor since it presents primarily at rest. It goes away with movement, but often returns when the limb, usually a hand or fingers, are held in one position. While holding a spoon or fork to the mouth, the tremor can reappear which is why those with Parkinsons are known to spill things. Parkinsons disease tremor may affect almost any part of the body, but most commonly involves the fingers, followed next most commonly by the hands, jaw, and feet.

A Parkinsons tremor differs from most other types of tremor because it is a resting tremor. It happens most often when a body part is relaxed rather than in motion. For a Parkinsons patient, a hand may quiver when resting in a lap or when holding a utensil to the mouth while eating.

The time course of improvement following high-frequency TMS differs from one person to another with PD. It also varies for different symptoms of PD. For instance, evidence shows that rigidity and resting tremors improve immediately, within a few seconds after high-frequency TMS.

Effects Of Rtms On Freezing Of Gait In Pd

About 50% of patients with Parkinsons disease experience freezing of gait at least twice a month. It is one of the diseases most common and debilitating symptoms, causing a person to become wheel-chair bound.

There is increasing evidence that a unique motor area of the brain plays a critical role in the disease process of FOG. Studies show that failure of this specific motor area to emit amplitudes sufficient to produce the intended forward stepping culminates in the FOG event. Hence, targeting this area via TMS can effectively alleviate this disabling symptom of PD.

A 2020 study published in the npj Parkinsons Disease Journal showed a statistically meaningful improvement in the FOG along with other biomarkers of Parkinsons disease in the group treated with rTMS.

Another study unveiled the benefits of five sessions of high-frequency rTMS on FOG in patients with PD. Not only did rTMS alleviate FOG in patients with PD, but these effects also persisted for a week post-rTMS. Moreover, it is seen that high-frequency rTMS can be a beneficial add-on tool for FOG just like it benefits other PD symptoms.
