- Catechol-O-methyltransferase inhibitors are relatively new medicines. They include tolcapone, entacapone and opicapone. These help to stop the breakdown of levodopa by the body, so more of each dose of levodopa can get into the brain to work. A COMT inhibitor is sometimes advised in addition to levodopa when symptoms are not well controlled by levodopa alone.
- Other medicines are sometimes used to help relieve symptoms. They have various effects which try to correct the chemical imbalance in the brain. They include beta-blockers, amantadine and anticholinergic medicines. One of these may be tried when symptoms are mild. However, you are likely to need levodopa or a dopamine agonist at some point.
Various things may influence which medicine is advised. For example, your age, severity of symptoms, how well your symptoms respond to treatment, if side-effects develop, other medicines that you may take, etc. Your specialist will advise on the best medicine for you to take. Whatever medicine or medicines you are prescribed, read the leaflet in the medicine packet for a full list of possible side-effects. Mention to your doctor if you develop a troublesome side-effect. Study To Examine Role And Value Of Parkinsons Specialist Nurses

Researchers in Northumbria are embarking on a £100,000 project aimed at getting a better understanding of the role of specialist nurses who support patients with Parkinsons disease.
Northumbria University is running the two-year nationwide research project The USP Project which is funded by Parkinsons UK, with Northumbria Healthcare NHS Foundation Trust.
They hope to develop an evidence base on the positive impact that specialist nurses have for Parkinsons patients and their families.

Those behind the project noted that Parkinsons is the fastest growing neurological condition in the world and lacked a cure. In the UK, 145,000 people are currently living with the condition.
They also highlighted the critical support provided by specialist Parkinsons nurses to people living with the condition, in terms of both medication, advice and information and emotional support.
In addition, according to figures from Parkinsons UK, specialist nurses save NHS commissioners a lot of money each year through these activities.
For example, a nurse can save a clinical commissioning group up to £43,812 in avoided consultant appointments, £80,000 in unplanned admissions and £147,021 in days spent in hospital on average.
Our research is designed to improve our understanding of the extent and value of specialist nurses working in the field of Parkinsons, she said.
Successful Parkinson’s Nurse Specialist Role Made Permanent For Vale Of York Patients
A specialist recruited to support people living with Parkinson’s disease in the Vale of York has proved a huge success in the pioneering role.
Gill Clark took on the mantle of Parkinson’s Nurse Specialist, working with patients registered with 14 GP practices in the South Hambleton and Ryedale, Tadcaster and Rural Selby and Selby Town Primary Care Networks, in July 2019.
NHS Vale of York Clinical Commissioning Group had agreed to a proposal by Parkinson’s UK for the charity to fund the role for two years and now, following an evaluation of Gill’s impact, the role has been made permanent by the CCG.
Gill offers expert care for people who have been diagnosed with Parkinsons Disease as well as their families and carers. She has specialist experience, knowledge and skills and is able to support them in a variety of ways, including referring them to appropriate services and therapies, working with GPs and hospitals and advising on medication.
The post was designed to address inequalities faced by the communities served by these Primary Care Networks, including access to transport and distance to services. The average distance to hospital for patients in South Hambleton and Ryedale was found to be more than 18 miles, while these three areas were also found to have high numbers of people living with Parkinson’s.

How Do Symptoms Progress And What Is The Outlook
The symptoms of PD tend to become gradually worse over time. However, the speed of progression varies greatly from person to person. When symptoms first begin, you may not need treatment when symptoms are relatively mild.
Most people with PD can expect to have some time of relatively mild symptoms. Then, when the symptoms become worse, they can expect several years of good or reasonable control of the symptoms with medication. But everyone is different and it is difficult to predict for an individual how quickly the disease will progress. Some people may only be slightly disabled 20 years after PD first begins, whereas others may be very disabled after 10 years.
Research into PD is active. For example, one main aim of research is to find medicines that prevent the damage to the affected cells, rather than just treating the symptoms, which is the main value of treatment at present. Further research on these chemicals continues. Research is underway using stem cell therapy to help treat PD. Other researchers are looking at alpha synuclein, a protein that gathers around the junction between nerve cells and is thought to affect the way messages are conducted between the brain and the nerves controlling movement.

Im currently the Parkinsons disease specialist nurse for NHS Western Isles. My role is quite varied, which I enjoy because each day is different.
A lot of my work is community based, so I go out to see people and assess them in their own homes. I also do clinics at the hospital occasionally.
Theres also Telemedicine every 5 weeks, where we link up with Glasgow and the consultant neurologist there to save people having to travel. Theres plenty of admin work, so I can still use my computer skills from many years ago!

Lastly, maintaining your relationship and communication with the person with Parkinsonâs can be the most challenging and rewarding aspect of caregiving. As Parkinsonâs disease progresses, the roles change and the person with Parkinsonâs may go from being an independent head of the household to a very dependent person requiring a significant level of care. However, research shows that despite high levels of strain, caregivers with good quality relationships have reduced depression and better physical health. Remember, as a caregiver your service to your loved one is beyond measure in terms of love, depth of care, and concern.
