Parkinsons is officially classified as a movement disorder because it involves damage to the areas of the brain, nerves and muscles that influence the speed, quality, fluency and ease of movement. Motor symptoms are usually the most visible elements of Parkinsons. Motor symptoms of Parkinsons also respond well to Parkinsons medications.

Although motor symptoms of Parkinsons often get the most attention and treatment, researchers estimate motor symptoms dont actually appear in most people until around 60?80% of the nerve cells in the brain that make dopamine have stopped working because of Parkinsons.

  • Take your medication on time! This helps minimize off times that can make tremor worse.
  • Streamline your focus to reduce multitasking. Rest your elbow on the table when taking a drink to stabilize yourself or sit down when you button your shirt to concentrate more intently on the task at hand.
  • Reduce stress. Nerves and feeling stressed, in general, can make tremor worse, as can fatigue.
  • Consider adaptive technologies and products designed to make everyday tasks easier, such as specially-designed razors, pens, keyboards, utensils, cups and dishes.

What Is A Resting Tremor

A Parkinsons tremor differs from most other types of tremor because it is a resting tremor. It happens most often when a body part is relaxed rather than in motion. For a Parkinsons patient, a hand may quiver when resting in a lap or when holding a utensil to the mouth while eating.

Most tremors are “action tremors” where the shaking happens when a person moves their muscles. These may occur when holding arms outstretched, holding a heavy item in one position, or reaching slowly and purposefully toward an object.

Tremor In Other Conditions

While tremor is a common symptom of PD, it can also be a symptom of other conditions, most notably essential tremor. The main difference between Parkinsons tremor and most other types of tremor is that in PD resting tremor is most common. Other conditions are usually characterized by action tremor, which tends to lessen at rest and increase when youre doing something, like trying to make a phone call or take a drink. Tremors of the head and voice are also common in essential tremor but rare in PD.

Tremor is not considered a life-threating condition. Although many cases of tremor are mild, tremor can be very disabling for other people. It can be difficult for individuals with tremor to perform normal daily activities such as working, bathing, dressing, and eating. Tremor can also cause social disability. People may limit their physical activity, travel, and social engagements to avoid embarrassment or other consequences.

The symptoms of essential tremor usually worsen with age. Additionally, there is some evidence that people with essential tremor are more likely than average to develop other neurodegenerative conditions such as Parkinsons disease or Alzheimers disease, especially in individuals whose tremor first appears after age 65.

Unlike essential tremor, the symptoms of physiologic and drug-induced tremor do not generally worsen over time and can often be improved or eliminated once the underlying causes are treated.

Tremors Yes Parkinson’s Disease No

Essential tremor is often mistaken for Parkinson’s disease, but the two conditions differ in several ways. Essential tremor is an action tremor, meaning that the involuntary shaking increases when you move and try to use your hands. In Parkinson’s disease, tremors occur mainly at rest, and activity reduces the symptoms. Some people with essential tremor develop head nodding or shaking few people with Parkinson’s do. Balance problems and rigidity of the arms and legs are common features of Parkinson’s disease but not of essential tremor. One of the hallmarks of essential tremor, useful in diagnosing the disorder, is that alcohol can temporarily ease symptoms in Parkinson’s, alcohol has little effect on the tremor.

Parkinson’s disease is marked by a progressive loss of brain cells that produce dopamine, a chemical messenger that enables normal body movements. Essential tremor also appears to involve a disruption in the activity of motor pathways, but it’s uncertain whether there is any loss of brain cells. Postmortem examinations of brain tissue taken from people with essential tremor reveal various abnormalities in the cerebellum and brainstem, including the loss of Purkinje cells, which produce an important neurotransmitter, called GABA. But the samples don’t all show the same changes, so the import of these findings is uncertain.

Diagnostic Criteria For Et & Pd

Two sets of criteria are commonly used for the diagnosis of ET. For definite ET, the diagnostic criteria proposed by the Movement Disorder Society require the presence of persistent, bilateral postural tremor of the forearms . Kinetic tremor may be present, but is not necessary for the diagnosis . No other abnormal neurological signs may be present, except for Froments sign, which is a cogwheel phenomenon without rigidity . For definite ET, the criteria proposed by the Washington Heights-Inwood Genetic Study of Essential Tremor require the presence of moderate amplitude postural tremor as well as kinetic tremor, with the latter resulting in impairment of activities of daily living .

The UK Parkinsons Disease Society Brain Bank criteria require postmortem confirmation for the diagnosis of definite PD . The diagnosis of probable PD requires bradykinesia and one of the following additional features: rigidity, 46-Hz rest tremor or postural instability . In addition, three supportive features are required . The National Institute of Neurological Disorders and Stroke criteria for PD only include clinical criteria for possible PD, which require three out of four of the following features: rest tremor, bradykinesia, rigidity or asymmetric onset .

What Is Parkinsons Disease

Parkinsons disease is a progressive condition that causes trouble with movement. Its caused by the death of cells in a part of the brain called the substantia nigra. This part of your brain produces the neurotransmitter dopamine.

In some cases, specific genetic mutations are linked to the development of Parkinsons. But more often, it seems to appear randomly. Its thought that environmental factors such as exposure to pesticides, herbicides, or air pollution may contribute, but more evidence is needed to understand the potential link.

According to the , Parkinsons most commonly develops in people over the age of 60 and affects men about 50 percent more often than women.

Medical Treatment Of Parkinsons Disease

Levodopa/carbidopa is the most effective medication in terms of improving the motor symptoms associated with Parkinsons disease. However due to the frequent, and sometimes serious side effects of this drug, neurologists prefer trying other medications first. Unfortunately, the medications available are not as good as levodopa/carbidopa in the control of the motor symptoms and also have some side effects that limit their use.

Selegiline a medication that does not have any major effects on the motor symptoms, might be the only medication with some protective effect of the nerve cells, but this is not yet well proven. The other medications are only used to treat the symptoms.

What Causes Essential Tremors

It is generally accepted that tremor is caused when problems arise in the thalamus, which is the structure of the brain that is responsible for coordinating and controlling muscle activity. However, at this time, researchers and medical professionals are not exactly certain what causes essential tremor.

Researchers are currently analyzing changes in specific chromosomes that have been linked to the development of essential tremor, but there have been no confirmed genetic connections as of yet.

In patients with a family history of ET, the specific manner in which essential tremor is inherited can vary. Most of the time, ET is inherited in an autosomal dominant pattern, which means that one altered copy of a gene can cause essential tremor. However, this inheritance pattern is not found in other families with ET, making it an unclear and unconfirmed connection to family history and genetics.

Misperception #: The Tremor Of Et Is Bilateral & Symmetric

Action tremor in patients with ET is usually, though not always, bilateral, and it is typically asymmetric . In a community-based study of 54 patients with ET, the use of clinical rating scales revealed on average a 1.32-fold sideside difference, and quantitative computerized tremor analysis revealed on average a 1.71-fold sideside difference in arm tremor severity . Unilateral arm tremor is less common, reportedly occurring in 2, 4.4 and 10% of ET cases, with values varying depending on the criteria used . One study evaluated unilateral arm tremor in 412 ET cases from 133 kindreds with presumed autosomal dominant ET . Inclusion criteria required unilateral kinetic or postural tremor for at least 5 years, without dystonic posturing or bradykinesia/rigidity . Only subjects with a first-degree relative with definite ET were included . Eighteen patients were identified as having isolated unilateral arm tremor without tremor affecting other body segments . Out of the 18 patients, 13 had a combination of postural and kinetic tremor and 5/18 had only unilateral postural tremor . Isolated unilateral postural tremor should raise the suspicion of PD, and the patient should be followed closely for the development of additional signs suggestive of PD.

Clinical pearl #3: Action tremor in ET is often but not necessarily bilateral. Small to moderate sideside differences are the rule rather than the exception.

Other Differences Between Parkinsons Disease And Essential Tremor

Several other factors can be used to distinguish between essential tremor and tremors from Parkinsons disease.

Family History

It is more likely for those with essential tremor to have a family history of the disease than those with Parkinsons. More than half of people diagnosed with essential tremor have a family member with the same diagnosis or a related condition. In fact theres a term for that familial tremor. On the other hand, fewer than 10 percent of people diagnosed with Parkinsons disease have it in their family tree.

Other Symptoms

In people diagnosed with essential tremor, the tremor is the primary symptom that they demonstrate. Parkinsons disease involves its unique tremors, but it is also characterized by other symptoms. Those others include slow movements and general slowness , stiff body motions and a stiff body overall, and balance or coordination problems.

Response to Alcohol

When people diagnosed with Parkinsons disease consume alcohol, they usually will not see a change in their tremors or other symptoms. Those diagnosed with essential tremor, however, often experience a temporary reduction in their tremors after drinking alcohol .

How Is Tremor Treated

Although there is no cure for most forms of tremor, treatment options are available to help manage symptoms. In some cases, a persons symptoms may be mild enough that they do not require treatment.

Finding an appropriate treatment depends on an accurate diagnosis of the cause. Tremor caused by underlying health problems can sometimes be improved or eliminated entirely with treatment. For example, tremor due to thyroid hyperactivity will improve or even resolve with treatment of thyroid malfunction. Also, if tremor is caused by medication, discontinuing the tremor-causing drug may reduce or eliminate this tremor.

If there is no underlying cause for tremor that can be modified, available treatment options include:

Medication

Focused Ultrasound

A new treatment for essential tremor uses magnetic resonance images to deliver focused ultrasound to create a lesion in tiny areas of the brains thalamus thought to be responsible for causing the tremors. The treatment is approved only for those individuals with essential tremor who do not respond well to anticonvulsant or beta-blocking drugs.

Surgery

When people do not respond to drug therapies or have a severe tremor that significantly impacts their daily life, a doctor may recommend surgical interventions such as deep brain stimulation or very rarely, thalamotomy. While DBS is usually well tolerated, the most common side effects of tremor surgery include dysarthria and balance problems.

Clinical Situation #: Pd Or Pd+et

Clinical summary

A 65-year-old man with rest tremor and rigidity in the right arm is diagnosed with PD. Five years later, he develops a postural tremor of his right arm, which occurs after a latency of 10 s and a frequency similar to his 4-Hz rest tremor.

A re-emergent tremor with similar frequency to the patients rest tremor is consistent with a diagnosis of PD. There are no additional features to suggest ET.

Finger Twitching

The pill rolling tremor that is often described in medical texts refers to the tremors of the fingers, usually the thumb plus the other fingers, that makes it look as if the person is rolling a pill in the fingers. This is most often the part of the body where tremors will begin.

Jaw Tremors

Jaw tremor is common in Parkinsons, whereas head tremor is not. The jaw tremor looks very much like shivering, although usually slower. It goes away with movement, so it is not present while chewing. It is extremely irritating when it is severe enough to cause the teeth to click together. For people with dentures, it may keep the dentures from staying in place, in addition to attracting attention because of the clicking sounds. Chewing gum stops the tremor.

Foot Tremors

Many, if not most people, may have a leg tremor if they rest their leg in particular positions with the heel on the ground. This is not a Parkinsons disease tremor. A foot tremor due to Parkinsons isa resting tremor, so it is most evident when the feet are dangling or when the person is lying down. It always disappears when the person is standing, and does not interfere with walking. The tremor usually involves the feet, but the thighs may also be involved so that the whole leg moves.

Can Et And Pd Occur Together

Even more frightening is when PD or other parkinsonism syndrome occurs along with ET. Parkinsonism refers to conditions that resemble PD in many ways but are not true PD. A big difference is that medications that work for PD, such as L-dopa, have little or no effect on parkinsonism. In addition, a person with parkinsonism may not have tremors but will have other similar PD symptoms: bradykinesia , stooped posture, stiff walking, small steps, and reduced arm swinging when walking. Treatment using Deep Brain Stimulation does not help with parkinsonism.

There are other variants that are considered atypical Parkinsons disease . There is also a very rare condition called Progressive Supranuclear Palsy . In its early stages it also looks like PD or one of the forms of parkinsonism but with ongoing degeneration, stiffness, dizziness and falls become worse. There can also be personality and cognitive changes. Since any of the above can co-occur with ET, gaining an accurate diagnosis of co-existing conditions can be a confusing process. Symptoms overlap, so trying to tease out one diagnosis from another is challenging.

What Are The Symptoms Of Essential Tremor

If you have essential tremor, you will have shaking and trembling at different times and in different situations, but some characteristics are common to all. Here is what you might typically experience:

  • Tremors occur when you move and are less noticeable when you rest.
  • Certain medicines, caffeine or stress can make your tremors worse.
  • Tremors may improve with ingestion of a small amount of alcohol .
  • Tremors get worse as you age.
  • Tremors dont affect both sides of your body in the same way.

Here are signs of essential tremor:

  • Tremors that are most obvious in your hands
  • Difficulty doing tasks with your hands, such as writing or using tools
  • Shaking or quivering sound in your voice
  • Uncontrollable head-nodding
  • In rare instances, tremors in your legs or feet
