The Substantia Nigra And Movement
The reason that Parkinsons causes movement symptoms is that the substantia nigra makes up part of the circuitry, called the basal ganglia, that the brain uses to turn thought about movement into action.
The structures of the basal ganglia.
The substantia nigra is the master regulator of the circuit, it mainly communicates using the chemical dopamine, but other chemical transmitters are also used to communicate between other areas of the basal ganglia.
The balance of signals being sent between these structures allows us to control movement. But as Parkinsons progresses, and the dopamine-producing brain cells in the substantia nigra are lost, movement symptoms appear. Without enough dopamine, it becomes harder to start and maintain movements, which leads to symptoms such as slowness of movement, rigidity and freezing. And an imbalance of signals in the basal ganglia means people with Parkinsons can experience what is known as a resting tremor.
But while this is the description of Parkinsons you may find in most textbooks, it is now recognised that changes are not limited to the substantia nigra and basal ganglia.
What Lifestyle Changes Can I Make To Ease Parkinsons Symptoms
Exercise: Exercise helps improve muscle strength, balance, coordination, flexibility, and tremor. It is also strongly believed to improve memory, thinking and reduce the risk of falls and decrease anxiety and depression. One study in persons with Parkinsons disease showed that 2.5 hours of exercise per week resulted in improved ability to move and a slower decline in quality of life compared to those who didnt exercise or didnt start until later in the course of their disease. Some exercises to consider include strengthening or resistance training, stretching exercises or aerobics . All types of exercise are helpful.
Eat a healthy, balanced diet: This is not only good for your general health but can ease some of the non-movement related symptoms of Parkinsons, such as constipation. Eating foods high in fiber in particular can relieve constipation. The Mediterranean diet is one example of a healthy diet.
Preventing falls and maintaining balance: Falls are a frequent complication of Parkinsons. While you can do many things to reduce your risk of falling, the two most important are: 1) to work with your doctor to ensure that your treatments whether medicines or deep brain stimulation are optimal and 2) to consult with a physical therapist who can assess your walking and balance. The physical therapist is the expert when it comes to recommending assistive devices or exercise to improve safety and preventing falls.

Beyond The Substantia Nigra
In Parkinsons, other areas of the brain beyond the substantia nigra are involved as the condition progresses. Changes in higher brain areas are linked to non-motor symptoms that can affect people with Parkinsons later on in the condition, and often have a significant impact on quality of life.
For instance, symptoms that affect memory and thinking can be linked to the presence of Lewy bodies in the largest area of the brain the cerebral cortexas well as the limbic system. The limbic system is also believed to be involved in symptoms involving mood and pain, and similar changes in the inferior temporal gyrus, an area of the brain involved in processing what we see, are thought to be the reason for hallucinations.
But research into the spread of Parkinsons through these areas, and how we can stop it , is just one side of the story. There is also ongoing research into where Parkinsons starts, and the effects it is having before it reaches these areas.
The presence of non-motor symptoms many months and maybe even years before the physical symptoms, such as tremor and slowness of movement, points towards the presence of other changes in the body long before the loss of dopamine-producing cells in the substantia nigra. These early symptoms could even help researchers predict those who will go on to be diagnosed with Parkinsons, which would help in the development of new and better treatments.
Stages Of Parkinsons Disease
Because Parkinsons disease develops over time, there are various stages that help identify how symptoms have progressed and what should be expected next.
Generally, doctors follow a set of five stages as outlined below:
The beginning stage of Parkinsons disease sometimes doesnt show any signs at all. If symptoms are noticeable, theyre usually tremors and affect one side of the body. The symptoms usually dont affect your daily routine, but they should be taken seriously and brought to the attention of your doctor, if they havent already.
During this stage, the disease starts to affect your whole body. The tremors and stiffness cause routine activities to take a little longer to complete, and your overall movement starts to be affected. This stage features a worsening of all the symptoms that started to progressively deteriorate in stage 2, but you also start to experience a loss of balance and coordination, as well as how quick your reflexes are. Difficulty Swallowing Or Eating
Parkinsons affects the muscles in the face, mouth, and throat that control speaking and swallowing. Dysphagia, or difficulty swallowing, is a symptom of Parkinsons that can lead to trouble eating.
It can lead to malnutrition, dehydration, or aspiration which happens when food or saliva goes down the wrong pipe and is inhaled into the lungs. Aspiration can lead to aspiration pneumonia, the leading cause of death in Parkinsons.

How Does This Condition Affect My Body
Parkinsons disease causes a specific area of your brain, the basal ganglia, to deteriorate. As this area deteriorates, you lose the abilities those areas once controlled. Researchers have uncovered that Parkinsons disease causes a major shift in your brain chemistry.
Under normal circumstances, your brain uses chemicals known as neurotransmitters to control how your brain cells communicate with each other. When you have Parkinsons disease, you dont have enough dopamine, one of the most important neurotransmitters.

Scientists have discovered gene mutations that are associated with Parkinsons disease.
There is some belief that some cases of early-onset Parkinsons disease disease starting before age 50 may be inherited. What Are The Symptoms Of Atypical Parkinsonian Disorders
Like classic Parkinsons disease, atypical Parkinsonian disorders cause muscle stiffness, tremor, and problems with walking/balance and fine motor coordination.
Patients with atypical Parkinsonism often have some degree of difficulty speaking or swallowing, and drooling can be a problem. Psychiatric disturbances such as agitation, anxiety or depression may also be part of the clinical picture.
Dementia with Lewy bodies can cause changes in attention or alertness over hours or days, often with long periods of sleep during the day. Visual hallucinations typically of small animals or children, or moving shadows in the periphery of the visual field are common in DLB. DLB is second only to Alzheimers disease as a cause of dementia in the elderly, and it most commonly affects patients in their 60s.
Patients with progressive supranuclear palsy may have difficulties with eye movements, particularly when looking downward, and with balance when descending stairs, for instance. Backward falls are common and may occur during the early course of the disease. PSP is not usually associated with tremor, unlike Parkinsons disease.

What Is The Quality Of The Reviewed Studies
Overall, quality scores were mediocre for both non-intervention and intervention studies. The main points that studies scored low on were sample size justification, electrode placement procedures and signal processing techniques. Individuals with PD exhibit great heterogeneity and generally high inter- and intra- subject gait EMG variability necessitating greater sample sizes than for HOA. However, the median sample size was only twenty-two and no study in this review performed power analysis to justify their selection of participant number. Most studies included a greater proportion of males, reflecting the gender bias in PD although some studies did not specify gender. Gender differences in muscle activity during walking have previously been reported, indicating it is an important factor. Only four studies determined electrode location using validated guidelines such as the SENIAM guidelines. Identification of the optimal electrode site helps ensure the signals with higher signal to noise ratio are recorded from the selected muscle with minimal cross-talk from adjacent muscles.
Over half of the studies did not report any signal normalisation methods,,,,,,,,. Such normalisation is essential to allow comparisons of EMG between muscles, sessions and participants as factors such as thickness of adipose tissue, presence of oedema and number and orientation of muscle fibres will modify amplitude,. Excluding normalisation can invalidate subsequent results.

Treatment Depends On Properly Identifying The Type
If pain is bilateral always assume it is central pain pain due to PD. In my experience Azilect works great for this type of pain. Other medications which can be employed for this pain as well.
Massage therapy works for all types of leg pain-my favorite therapy but can be costly. Water therapy may also work for all types except central pain. Physical therapy can alleviate dystonia pain, as well as musculoskeletal and radicular pain.
If pain is due to dystonia related to levodopa intake, find out when it occursend of dose or at peak dose. Typically adjusting medication doses will resolve problem. However, if dystonia is an initial symptom of PD, initiating treatment with levodopa will resolve. If medication adjustment does not work well for levodopa induced dystonia, another treatment option is DBS . Pain due to dystonia independent of cause can also respond well to Botox injections, as well as centrally acting muscle relaxants. To avoid and alleviate pain caused by stiff muscles, a great treatment option is activity in the form of stretching exercisesany number of activities will do such as tai-chi or yoga. For me when I start having radicular pain shooting down my leg it is time to up my levodopa dosage.
