If you have movement or speech problems because of Parkinsons disease, a specialized therapy program can help.
LSVT BIG and LOUD® therapy was developed for people with Parkinsons disease. It also can be used for people who have other neurological conditions. LSVT programs are administered in an intensive manner four times per week for four weeks and are tailored to each patient.
The emphasis is on exaggerated activity to compensate for PDs movement and speech symptoms. For movement issues, therapists ask patients to move in BIG or amplified ways, whether theyre walking, buttoning shirts and pants, getting keys or change out of pockets, or writing. For speech, the focus is on a single goal: LOUD speaking. Activities may include sustaining a vowel sound such as ah, oral reading and speaking activities that increase in complexity .
The majority of PD patients I send for therapy find it to be extremely helpful, says Kelvin L. Chou, M.D., professor of neurology and neurosurgery at Michigan Medicine and co-director of the STIM Program.
How Hard Should I Exercise If I Have Parkinsons Disease
A rating of perceived exertion is a good way to measure intensity. On a scale from 0 to 10, 0 would be how you feel while sitting or lying down, while 10 would be the maximum effort you can give. Building up to an effort between 5 to 8 means you are exercising at a high intensity. A good gauge is, if you can have a conversation with someone while exercising, you should probably increase your intensity.

Big Exercises Have Big Impact On Parkinsons

As many as 1 million Americans live with Parkinsons disease, a brain disorder that may lead to tremors, stiffness and difficulty with walking, balance and coordination. Although are diagnosed with Parkinsons each year, thousands of cases go undetected.
Symptoms of Parkinsons disease usually begin gradually and worsen over time. Common include tremors or shaking trouble moving or walking stooping or hunching over and a soft or low voice. Many patients in the early stages of the disease think their symptoms are due to normal aging and do not seek help from a doctor.
Once patients do seek help, diagnosis can be difficult. There are no medical tests to diagnose Parkinsons with certainty, and many other conditions may produce similar symptoms. Once it is confirmed that a patient has the disease, it is important that they begin therapy as soon as possible.
Our Rehabilitation Center works directly with doctors to get referrals as soon as patients are diagnosed, says Pamela Singh, physical therapist at Sharp Grossmont Hospital. This increases the likelihood that the treatment will slow progression and decrease symptoms of the disease.

What Kind Of Exercise Can I Do If I Have Trouble Standing Or Walking
Even with advanced Parkinsons symptoms, you can still reap the benefits of some activities. If you have trouble walking or balancing, hold a bar or rail to exercise and stretch. If standing or getting up is tough, exercise and stretch in a chair or bed. Physical exercise performed in a seated position, such as biking on a recumbent bike can allow you to exert yourself in a safe manner.
Facial exercises may help combat difficulties speaking or swallowing:
- Chew your food longer and more vigorously.
- Exaggerate your face and lip movements when you speak.
- Make faces in the mirror.
- Sing or read out loud.
Mental exercises give your brain a workout and can improve memory. For example:
- Name as many animals as you can in 1 minute.
- Play brain games and do puzzles.
- Solve math problems in your head.
You can also add activity in small bits throughout your day:
- Park further away from stores so you walk longer distances.
- Stretch or do leg exercises while watching TV.
- Swing your arms more when you walk, and take long strides.
- Take the stairs instead of the elevator.

Chair Exercise 1 Improve posture in patients living with Parkinsons.
Sit in a stable chair with your back against the base.Lean forward and reach with your hands toward your feet out in front of you.Quickly and with high energy, pull back into your original seating position with your back flat against the chair.Repeat several times.
Chair Exercise 2 Regain rotation of the trunk to counter the effects of Parkinsons disease.
Sit comfortably in a stable chair and place feet shoulder-width apart.Place your hands out in front of you, with both palms touching.Take one arm and stretch out to your side, leaving the other hand at the center. Be sure to extend your arm with your fingertips are engaged, so you can obtain maximum efficiency.Quickly and with high energy, bring your stretched arm back to the center and smack the palms of your hands.Repeat these motions several times on both hands.

Are There Any Risks Of Exercising With Parkinsons Disease
Some symptoms, like Parkinsons tremors, may seem worse during exercise. But exercise generally improves tremors and other symptoms in the long run.
Reduce challenges by stretching before and after exercise. Use good form to prevent injury. And avoid slippery floors, poor lighting and tripping hazards. If you have pain, stop and rest.
Pushing yourself too hard during exercise can lead to injury. Start slowly and increase intensity and duration over time. Keep a log to track your exercise choices and how you feel. Eventually, youll learn what works best for you.

The BIG part of the program focuses on increased large limb movements making improvements to balance, speed of movement and overall quality of life. The BIG therapy, which has been developed over the last 20 years, incorporates proper exercise techniques to engage big limb movements with core strengthening.
