Neuroprotection is when your brain works to prevent the death of neurons, or brain cells. For people with PD, exercise is not only vital to maintaining balance, mobility and daily living activities, but it has the potential to have a neuroprotective effect.
The Parkinsons Foundation studied exercise as part of our Parkinson’s Outcomes Project study. Our Center of Excellence network believes that exercise is important to good outcomes in PD, and data supports that. Exercising enhances the sense of wellbeing, even across different disease stages and severities.
How Do We Know Exercise Helps People With Parkinsons
We keep telling you that exercise is important . But what is the evidence that these exercise techniques really help people with PD? Lets review three of these types of exercise tai chi, yoga and dance and look at the data as to how effective they are for people with PD.
Tai chi is an ancient Chinese form of martial arts, which today is practiced as a style of exercise combining flowing movements, breathing techniques and meditative practices. Tai chi is low impact and is suitable for all ages and levels of fitness. It does not require any special equipment.
There has been research focused on the general health benefits of tai chi, and these may include reduction of stress, anxiety and depression, as well as improved flexibility, balance, agility, strength and stamina.
These are just three types of exercise that show promise and benefit for people with PD. So go ahead and try an online dance, yoga or tai chi class at home . Or you can try another exercise type that piques your interest and start moving!

STARTING POSITION: Lying on your side. The leg closest to the floor is bent, and the top leg is straight. Cradle your head with your arm, or use a pillow for neck support.

Its very easy to do this exercise incorrectly. When done correctly, you should feel muscles in the back of your hip working. You should NOT feel it in the front of your hip. If you do, youre using your hip flexor or TFL muscles.
- Let your hips fall backward. Keep them stacked by actively leaning forward.
- Let your leg kick in front of you. Raise it straight up as if youre sliding your heel along an invisible wall behind you.
- Let your toes point up. Keep your foot parallel to the floor, or even point the toes slightly down to the floor if possible.

Which Types Of Exercise Are Best For Parkinsons
It is important to state upfront that there is no one best type of exercise for people with PD. It is most important to choose an exercise regimen that you enjoy, and will continue to do.
However, beyond doing exercise that you will stick with, there are some additional concepts to consider when designing an exercise program for someone with PD.
Impact Of Exercise On The Brain
Dopamine is a neurotransmitter that controls the brains reward and pleasure centers, as well as regulating movement and emotional responses. In PD, there is a dopamine deficiency.
While exercise has not been shown to increase the amount of dopamine in the brain, research does show that it helps it to be used more efficiently.
The exercises that someone with PD should undertake will depend on how limiting their symptoms are. In all cases, the exercise should focus on three areas:
- flexibility and stretching
- aerobic, also known as cardio
- resistance or using the muscles against opposing force
There are many different types of exercise that involve all three of these areas. These exercises include:
The exercise type known as random practice has been shown to benefit people with PD particularly. This is an aerobic exercise that challenges the individual to change speed, activity, or direction.
It is also essential for someone with PD to vary activities. This is because people with the condition may have trouble changing activity and doing two activities at the same time. As a result, random practice and variation will help to challenge those symptoms.

Fitness Tips To Manage Pd Symptoms
- Choose an exercise program that you will actually do! Dont design a great, Parkinsons-specific program and then skip it because its too hard or not fun.
- Follow a varied routine. Perform simple stretches and posture exercises daily, and make sure to include aerobic and strengthening exercises several times per week.
- Keep intensity at a level that feels somewhat hard for you.
- Consider joining an exercise class or group. Classes are good motivation and also provide an opportunity to socialize. Trained instructors give clear guidelines and offer modifications.
- Try exercise videos or home exercise equipment if it is difficult to get out.
- Music can enhance performance by providing rhythm to coordinate movement.
- Be creative with your fitness. Challenge yourself and have fun!

Information Sources And Study Selection
We systematically searched Medline , Embase, PsycInfo, CINAHL, and CENTRAL for the term Parkinsons in combination with widely used terms describing cognition-based intervention from inception to November 6, 2014. There was no limit on publication language. Reference and citation lists of relevant studies were manually scanned for potential eligible articles. One reviewer performed initial eligibility screening by assessing titles and abstracts of all results. Following initial screening, 2 independent reviewers assessed full-text versions of potentially eligible articles.

Data Extraction And Quality Assessment
Two investigators independently extracted the relevant data and information from the eligible studies. Basic information about the study characteristics , population, interventions and comparisons , and measurement outcomes was extracted. Another two reviewers evaluated the quality of the included studies based on the standard criteria of the Cochrane Risk of Bias Tool . Any disagreements that existed were resolved through discussion. If necessary, a senior investigator was consulted to achieve a consensus.

Effects Of Physical Exercise On The Treatment Of Parkinsons Disease
Other therapeutic strategies have been evaluated clinically and scientifically in recent years in the search for an action to reduce clinical problems of PD, such as, non-pharmacological interventions like physiotherapy and physical exercise . Rehabilitation through physical therapy has a variety of goals and methods that generally promote benefits in parkinsonian mobility, posture, and balance. However, some limitations have been observed in a consensual way by some researchers in two topics: in relation to the benefits that seem to be more immediate , and the variety and low methodological quality of the studies . Other nonpharmacological approaches to rehabilitation in Parkinsons disease are the practice of different modalities of physical exercises such as walking, running, strength training, whole body vibration and functional exercises, which are related to the reduction in the risk of falls, decreased motor symptoms, motor performance improvements, balance and gait improvements, positive repercussions in quality of life and executive functions .
Kurtais et al. investigated the effects of six weeks of supervised treadmill walking, three times a week for 40 minutes in patients with mild to moderate PD, and observed significant improvements in lower limb functional parameters such as walking, balance, and agility, and in related parameters, the adaptations promoted by aerobic exercise as increase of peak VO2 and caloric expenditure in METs .

When trying to find an exercise class to join, it can be hard to know which ones to try because there are many different types of classes that are marketed to people with PD. When you are evaluating a class, it is important to remember the following:
- It is the components of the exercise class that are important . You do not need to specifically do boxing or dance if you can achieve these elements of exercise in other ways.
- The instructor should have expertise in PD. APDA offers a training for fitness professionals which helps them better understand how to create exercise programs best suited for people with PD.
- The ratio of class members to instructors should be low enough to maintain safety.
- The class should be adaptable for people of different abilities and mobility. Many classes will offer seated options or other modifications.
It may take a few tries to find the types of exercise that suits you best so dont give up! Feel free to try different classes in your area to see what types of exercise appeal to you. It can also be motivating to take classes together with a friend or family member. You can hold each other accountable, and also have some fun together. A rating of perceived exertion is a good way to measure intensity. On a scale from 0 to 10, 0 would be how you feel while sitting or lying down, while 10 would be the maximum effort you can give.
