What Type Of Exercise Should I Do If I Have Parkinsons Disease

Exercise is a planned, structured, repetitive activity that is intended to improve physical fitness. There is no right exercise for people with Parkinsons. Everyones regimen will differ, depending on overall health, symptoms and previous level of activity. Any exercise helps, and a variety of exercise types may provide well-rounded benefits.

Aerobic exercise

Aerobic exercise involves activities that challenge your cardiorespiratory system such as walking, biking, running, and activities in the pool. Participating in aerobic exercise at least three days a week for 30-40 minutes may slow Parkinsons decline.

Strength training

Strength training involves using your body weight or other tools to build muscle mass and strength. Strength training two days per week, starting with low repetition and weight, may be beneficial in Parkinsons disease. A focus on extensor muscles, or muscles in the back of the body, can help with posture.

Flexibility training

Stretching two or more days per week can be beneficial to maintain range of motion and posture. Holding each stretch of major muscle groups for 30 to 60 seconds can improve muscle length.

Balance and agility training

This type of training often combines aerobic exercise, strength training, and flexibility training. Examples include:

  • Tai chi, yoga or Pilates.

Lsvt Loud Outcome Data

Two randomized controlled trial studies have been conducted . Data have documented that training increased vocal loudness results in a statistically significant and lasting increase in vocal sound pressure level and frequency variability during speech as compared to a matched treatment focusing on training increased respiratory support . Effect size data for the primary outcome variable of vocal SPL in conversational speech were highly significant immediately posttreatment and were maintained at 24 months posttreatment . Data providing initial external validation of LSVT LOUD outcomes have been reported by independent labs and reviews .

In addition, various physiologic changes such as increased movement amplitude of the rib cage during speech breathing , increased subglottal air pressure , and improved closure and larger/more symmetrical movements of the vocal folds have been documented in individuals with PD immediately after LSVT LOUD. These findings are supported by perceptual data demonstrating listeners rated improved loudness and voice quality in individuals with PD immediately posttreatment . Subjects in these studies were predominately Hoehn and Yahr stages 13 with moderate speech deficits.

How To Start Exercising If Youre Living With Parkinsons

Safety is key. The first thing you need to do is talk with your neurologist and primary care doctor to make sure that the exercise regimen that you embark upon is safe for you.

Next, ask for a referral for physical therapy. A physical therapist will be able to figure out what movement challenges you may have and design a program to help you improve. There are certain physical therapists with additional training in Parkinsons. Your physical therapist will work with you for your allotted sessions, and then can help you plan your ongoing exercise regimen that is tailored to you. You can contact the APDA National Rehabilitation Resource Center for Parkinsons Disease for help finding resources in your area.

Additionally, physical therapy can help counteract the tendency for people with PD to reduce the size of their movements. Weight And Resistance Training

It is known that the extensor system the muscles that keep the body upright against gravity become weaker in Parkinsons, making it more likely that the body will become stooped and flexed. Lifting weights or using resistance equipment can be the simplest way to work on muscle strength, however it is important you know what you are doing so you dont hurt yourself, and so that you strengthen the right set of muscles.

These exercises normally focus on building strength in sets or muscles or improving certain movements. And can often be done at home without the need of expensive equipment. If you are interested in doing weight and resistance exercises we advise getting professional advice about what muscles or movements to work on from a qualified physiotherapist.

There is evidence that this type of exercise can improve strength and flexibility in Parkinsons. For instance, resistance training focusing on the legs was seen to improve legstrength and mobility of the knee joint in people with Parkinsons. But the researchers in this study noted it may not be superior to treadmill or balance training, which provided many of the same benefits.

Other researchers have demonstrated that a combined strengthening, stretching and balance training programme, carried out 2 days a week over 24 months, may be more effective than weight lifting alone in reducing Parkinsons motor symptoms.

Tips For Getting Started

  • First, be safe. Before starting an exercise program, consult your neurologist and primary care doctor about concerns and recommendations.
  • Ask your doctor or members in your support group to refer to a physical therapist who knows about PD. Work together to identify your concerns and limitations. Target exercises to improve them. For most people, a structured exercise program will include aerobic exercise and resistance training .
  • Purchase a pedometer and figure out how many steps you take on average each day, then build up from there. Many smartphones or smartwatches have a built-in pedometer feature or an application that can be downloaded.
  • Exercise indoors and outdoors. Change your routine to stay interested and motivated.
  • Again, most importantly pick an exercise you enjoy.

This exercise program attacks Parkinsons at its vulnerable neurological points. While focusing on overall fitness, workouts include: focus mitts, heavy bags, speed bags, double-ended bags, jump rope, core work, calisthenics and circuit weight training. No boxing experience is necessary and people of all ages are invited to participate. A Cornerman is required to assist any participants who answer yes to any of the following: requires an assistive device to walk with, has significant balance issues and/or has had more than one fall in the past 3 months, has difficulty following directions without repetition.

Are There Any Parkinsons Specific Exercises

There are a number of Parkinsons specific programmes that aim to improve movement. Overall there is limited research evidence on the specific benefits of these programmes, and we dont yet know if they are more beneficial than other types of exercise, but people have told us they found them to be of use.

We have provided examples below of the more common programmes specific to the needs of people with Parkinsons, but you may find classes with different names that are run by physiotherapists near you and offer similar exercises by trained therapists.

LSVT®BIG training is administered over the course of a month by a qualified physiotherapist or occupational therapist. It involves repetitive intensive high-amplitude movements and aims to restore normal movement amplitude. Studies into LSVT BIG training have been small in size but there is some evidence it may improve daily activities and motor symptoms.

It is claimed that this training provides greater improvements in motor performance than either Nordic walking or non-supervised in-home exercises. However, another study found that an 8-week Nordic walking course and a 4-week LSVT BIG course produced similar improvements in reaction time.

PD Warrior is exercise treatment designed to slow Parkinsons down. It was developed by Australian physiotherapists but practitioners can now be trained in the UK. Its a complete rehab program designed to improve your function, quality of life and long term exercise behaviour.

To get started with the LSVT BIG program, you first need a prescription for the program from your doctor. This may come from your primary care physician or your neurologist. Then contact a facility that has LSVT BIG trained therapists. You can search for an LSVT Certified Clinician near you on the LSVT Global website.

1. Maintaining your balance

To maintain balance, patients of Parkinsons disease can practice swinging both arms while walking. This will lessen fatigue and loosen the arms and shoulders. Furthermore, walking while changing the speed of your gait from fast to slow from one destination to another is a good technique for improving your balance. Using a chair as a support, you can also do leg lifts to the front and side, making sure that your back stays straight and your spine remains in a neutral position.

2. Walking

The best exercise to improve your walking in Parkinsons disease is toes up! The rule is to stride forward, striking the heel and rolling the foot as you transfer weight forward to the toe. When Is The Lsvt Big Program Appropriate

Patients with Parkinsons Disease can participate in the LSVT BIG program at any time during their disease process. However, research suggests that the earlier a patient gets into the program, the more beneficial it can be. The exercise program can be modified to accommodate patients at different levels of mobility.

The Basic Elements Of Exercising With Parkinsons

There are four core elements of exercise that are important for people with PD:

  • Aerobic e.g. brisk walking, stationary cycling activities that get the heart pumping
  • Strengthening e.g. using weights or resistance bands to improve muscle strength
  • Balance e.g. tai chi, dance to help you be more steady on your feet
  • Stretching e.g. mat exercises, yoga to provide flexibility
  • Including all four of these elements in your exercise regimen is ideal .

    Aerobic activity or high-intensity exercise may be particularly important for Parkinsons and general health

    High-intensity exercise has been formally studied in PD with impressive results. The Study in Parkinson Disease of Exercise Phase 2 enrolled 128 people with early PD, who were not yet on dopaminergic medication into three groups:

    • a high-intensity treadmill exercise group, in which people exercised at 80-85% of their maximum heart rate
    • a moderate-intensity treadmill exercise group, in which people exercised at 60-65% of their maximum heart rate
    • a wait-list control group

    After six months, the high-intensity group had essentially no change in their motor scores, whereas the control group had a three-point worsening of their motor scores.

    Currently, the SPARX3 trial is enrolling participants and underway. This trial is similar to SPARX2, but with a goal of studying many more participants.
