A review of existing research published in the August 2016 edition of the journal Frontiers in Medicine found that exercise may also improve cognition, while reducing depression and fatigue.

What is the one exercise that is best? As an Exercise Scientist with a background in strength and conditioning I hear this A LOT. And what is typically meant by this question is, ‘what is the ONE exercise I can do in the shortest amount of time that will fix all that ails me and will make me look good?’ While we all want a magic exercise pill we know there is no such thing. And there is also no one exercise that will address the functional changes associated with having Parkinson’s disease. Decades of data consistently show that an all-inclusive exercise regimen that focuses on muscular strength, cardiovascular fitness, balance and flexibility is key for overall health and fitness, particularly for those with a progressive disease like Parkinson’s.
Importantly, it is not just aerobic activity and cardiovascular fitness that we need to be mindful of. As one ages, muscular strength and muscular flexibility decline resulting in decreased range of motion and less ability to perform common tasks like carrying groceries, walking long distances or up stairs. Due to this, the evidence based guidelines recommend doing muscle strengthening on the major muscle groups 2 or more times per week.
What Exercises Are Best For People With Parkinsons
Researchers are looking at different types of exercise, including tai chi, tango dancing, gym and resistance training. Often, exercise studies do not compare different types of exercise side-by-side, so there is no definitive answer to which type of exercise is best.
However, at Parkinsons UK we know how important it is to have guidance on the best exercise for Parkinsons. So weve been working with leading physiotherapists and built on their extensive clinical experience to develop recommendations about what exercise is most suitable for people with Parkinsons.
There are different exercise styles depending on how your symptoms affect you. And everyone with Parkinsons should be looking for exercise that help them to do:
- aerobic activity to get the heart pumping
- exercises that concentrate on strengthening the major muscle groups
- regular stretches that move limbs through thefull range of motion
- exercises that focus on, and improve, balance, and
- combined movements that make you think hard as you co-ordinate your four limbs together, or that work your ability to do two or more things at once can play an important part of your exercise routine.

What Kind Of Exercise Can I Do If I Have Trouble Standing Or Walking
Even with advanced Parkinsons symptoms, you can still reap the benefits of some activities. If you have trouble walking or balancing, hold a bar or rail to exercise and stretch. If standing or getting up is tough, exercise and stretch in a chair or bed. Physical exercise performed in a seated position, such as biking on a recumbent bike can allow you to exert yourself in a safe manner.
Facial exercises may help combat difficulties speaking or swallowing:
- Chew your food longer and more vigorously.
- Exaggerate your face and lip movements when you speak.
- Make faces in the mirror.
- Sing or read out loud.
Mental exercises give your brain a workout and can improve memory. For example:
- Name as many animals as you can in 1 minute.
- Play brain games and do puzzles.
- Solve math problems in your head.
You can also add activity in small bits throughout your day:
- Park further away from stores so you walk longer distances.
- Stretch or do leg exercises while watching TV.
- Swing your arms more when you walk, and take long strides.
- Take the stairs instead of the elevator.
Tips For Exercising Safely
Before starting an exercise program, consult with your neurologist and primary care doctor about any health concerns and ask for recommendations, the Parkinsons Foundation advises.
Ask your doctor to refer you to a physical therapist who knows about Parkinsons; together, the two of you can identify any concerns and physical limitations you may have. Your exercise regimen should be targeted to address your symptoms and physical limitations.
You should stop any exercise or stretch that causes pain, and take steps to prevent falls while exercising, such as:
- If indoors, remove area or throw rugs
- Work out in well-lit areas
- Dont use rolling chairs
- Work out with friends or buddies, particularly when performing outdoor activities
- Stay hydrated
- Avoid overexertion

With Parkinsons, the brain neurons that make the neurotransmitter dopamine become damaged. The resulting drop in dopamine causes the classic symptoms of tremors, rigidity, slowing of movements, and impaired balance and coordination. The disease usually affects people over the age of 60, but young-onset Parkinsons disease can occur as early as your 30s.

Can Parkinsons Be Reversed With Exercise
While exercising cannot reverse the onset of Parkinsons disease, it can help in slowing the progression of the disease. According to the;Parkinsons Outcomes Project, people with a Parkinsons diagnosis who start exercising earlier and for a minimum of 2.5 hours a week have experienced a slower decline in quality of life compared to those who begin exercising later after the diagnosis.;

According to the APDA, exercising in water is a safe and effective way to improve balance and strength in people with Parkinsons disease. In general, the organization says, buoyancy from simply standing in the pool can help support weaker muscles and improve a persons balance and posture.

It is important to state upfront that there is no one best type of exercise for people with PD. It is most important to choose an exercise regimen that you enjoy, and will continue to do.
However, beyond doing exercise that you will stick with, there are some additional concepts to consider when designing an exercise program for someone with PD.;
How Patients Are Using Cycling To Slow Down Parkinson’s
Parkinsons symptoms include tremor, rigid muscles and problems with movement. While early treatment can delay the worst symptoms, people almost always get worse. About 60,000 Americans are diagnosed with Parkinson’s disease each year and about a million Americans have Parkinsons now.
No medical therapy can cure Parkinsons and while exercise was always shown to help people feel better, it was not generally accepted as a true therapy until recently.
Now teams are trying to find out how much exercise helps and just which symptoms it affects. Doctors say theyd be thrilled just to slow the inevitable worsening of the disease and if they can freeze progression or reverse symptoms, that would be a home run.
Corcos and colleagues say the most intense exercise appears to have at least temporarily frozen symptoms in many of their volunteers.
“The earlier in the disease you intervene, the more likely it is you can prevent the progression of the disease,” Corcos said in a statement.
“We delayed worsening of symptoms for six months, he added. Whether we can prevent progression any longer than six months will require further study.”

They worked with 128 patients with early stage Parkinsons. They randomly assigned them to either moderate exercise four days a week, intense exercise four days a week, or no additional exercise.
“This is not mild stretching. This is high intensity, Corcos said.

People diagnosed with Parkinsons can take multiple steps to slow the progression of the disease via medication, surgery, and lifestyle changes. Neurologists recommend exercise as an effective way to slow symptom progression due to the positive effects physical activity can have on the brain.;
The Basic Elements Of Exercising With Parkinsons
There are four core elements of exercise that are important for people with PD:
Including all four of these elements in your exercise regimen is ideal .
Aerobic activity or high-intensity exercise may be particularly important for Parkinsons and general health
High-intensity exercise has been formally studied in PD with impressive results. The Study in Parkinson Disease of Exercise Phase 2 enrolled 128 people with early PD, who were not yet on dopaminergic medication into three groups:
- a high-intensity treadmill exercise group, in which people exercised at 80-85% of their maximum heart rate
- a moderate-intensity treadmill exercise group, in which people exercised at 60-65% of their maximum heart rate
- a wait-list control group
After six months, the high-intensity group had essentially no change in their motor scores, whereas the control group had a three-point worsening of their motor scores.
Currently, the SPARX3 trial is enrolling participants and underway. This trial is similar to SPARX2, but with a goal of studying many more participants.

Choosing The Right Exercise Class For Parkinsons
When trying to find an exercise class to join, it can be hard to know which ones to try because there are many different types of classes that are marketed to people with PD. When you are evaluating a class, it is important to remember the following:
- It is the components of the exercise class that are important . You do not need to specifically do boxing or dance if you can achieve these elements of exercise in other ways.
- The instructor should have expertise in PD. APDA offers a training for fitness professionals which helps them better understand how to create exercise programs best suited for people with PD.
- The ratio of class members to instructors should be low enough to maintain safety.
- The class should be adaptable for people of different abilities and mobility. Many classes will offer seated options or other modifications.
It may take a few tries to find the types of exercise that suits you best so dont give up! Feel free to try different classes in your area to see what types of exercise appeal to you. It can also be motivating to take classes together with a friend or family member. You can hold each other accountable, and also have some fun together. As you find classes you enjoy, you can incorporate those modalities into your exercise schedule.
