Genes implicated in Mendelian forms of PD have provided new insights into the pathogenesis of the disease. The molecular pathways identified in monogenic cases may also be implicated in sporadic PD. The effect of dosage of SNCA on the phenotype of patients with duplications or triplications is illustrative. In addition, non-coding variants in this gene, thought to affect the level of expression in neurons, are associated with risk of the disease. The molecular mechanisms that contribute to PD and related disorders result in the death of dopaminergic neurons in vulnerable brain regions, and consequently the shared phenotype. However, known PD-causing genes account for only a small fraction of monogenic forms. Robust high-density SNP genotyping technologies and data analysis programs, combined with the analysis of copy number variations and large pathogenic genomic rearrangements, will identify novel loci. The clinical heterogeneity of parkinsonism is probably the cumulative effect of different gene-environment and/or genegene interactions. To identify risk variants in PD, association study methodology must be improved. Studies in isolated and heterogeneous populations, and approaches that minimize population stratification, are needed. Large-scale studies and publicly available GWA databases, crucial for statistical power, require collaborative efforts with shared sets of stringent clinical, genetic and analytic methods.

Dr. Shprecher also added that maintaining a healthy diet and getting regular exercise can help improve quality of life.;Physical and speech therapists;are welcome additions to any caregiving team.

The Facts About Parkinsons Disease
Parkinson’s disease is a progressive neurogenerative disease that causes nerve cells in the area of the brain that controls movement to weaken and/or die. While healthy neurons produce a chemical called dopamine, which the brain needs a certain amount of in order to regulate movement, weakened neurons produce lower levels of dopamine. What causes these neurons to weaken is currently unknown.;
Some patients with Parkinson’s disease also suffer from a decline in norepinephrine, a chemical that transmits signals across nerve endings and controls various functions, such as blood pressure and heart rate.
More than 10 million people worldwide are currently living with Parkinson’s disease and nearly one million will be living with the disease in the United States this year, according to the Parkinson’s Foundation.
Is Parkinsons Disease Inherited
Scientists have discovered gene mutations that are associated with Parkinsons disease.
There is some belief that some cases of early-onset Parkinsons disease disease starting before age 50 may be inherited. Scientists identified a gene mutation in people with Parkinsons disease whose brains contain Lewy bodies, which are clumps of the protein alpha-synuclein. Scientists are trying to understand the function of this protein and its relationship to genetic mutations that are sometimes seen in Parkinsons disease and in people with a type of dementia called Lewy body dementia.
Several other gene mutations have been found to play a role in Parkinsons disease. Mutations in these genes cause abnormal cell functioning, which affects the nerve cells ability to release dopamine and causes nerve cell death. Researchers are still trying to discover what causes these genes to mutate in order to understand how gene mutations influence the development of Parkinsons disease.
Scientists think that about 10% to 15% of persons with Parkinsons disease may have a genetic mutation that predisposes them to development of the disease. There are also environmental factors involved that are not fully understood.

Two Forms Of Parkinson’s Disease Identified
- Why can the symptoms of Parkinsons disease vary so greatly from one patient to another? A consortium of researchers is well on the way to providing an explanation. Parkinsons disease is caused by a protein known as alpha-synuclein, which forms aggregates within neurons, killing them eventually. The researchers have succeeded in characterizing and producing two different types of alpha-synuclein aggregates. Better still, they have shown that one of these two forms is much more toxic than the other and has a greater capacity to invade neurons.
Environmental Factors And Exposures
Exposure to pesticides and a history of head injury have each been linked with PD, but the risks are modest. Never having smoked cigarettes, and never drinking caffeinated beverages, are also associated with small increases in risk of developing PD.
Low concentrations of urate in the blood is associated with an increased risk of PD.

Theory Of Pd Progression: Braaks Hypothesis
The current theory is that the earliest signs of Parkinson’s are found in the enteric nervous system, the medulla and the olfactory bulb, which controls sense of smell. Under this theory, Parkinson’s only progresses to the substantia nigra and cortex over time.
This theory is increasingly borne out by evidence that non-motor symptoms, such as a loss of sense of smell , sleep disorders and constipation may precede the motor features of the disease by several years. For this reason, researchers are increasingly focused on these non-motor symptoms to detect PD as early as possible and to look for ways to stop its progression.

Does Parkinsons Lead To Dementia
During the final stage of Parkinsons disease, your symptoms may progress from severe motor function inhibition to memory loss. Generally, any form of memory loss is known as dementia.
There are multiple forms of dementia, such as:
- Alzheimers disease
- Huntingtons disease
- Dementia caused by head trauma
- Dementia caused by alcohol and drug abuse
Parkinsons disease is listed as a form of dementia simply because memory loss occurs in its later stages. Its estimated that memory loss doesnt become a concern until at least 10 years after the initial diagnosis. While memory loss isnt a cardinal symptom of the disease, more than half of Parkinsons patients develop some form of dementia. Like Alzheimers disease, the dementia associated with Parkinsons disease progressively grows worse.
The Alzheimers Association says that if dementia does stem from Parkinsons disease, the symptoms are similar to dementia with Lewy bodies. This is because patients start to develop these protein deposits on the parts of their brain that affect movement and cognition, which can then affect memory.

Why 2 Types Of Parkinsons Disease Are Plausible
Study coauthor Per Borghammer, MD, PhD, a professor of clinical medicine at Aarhus University, tells Verywell that its unclear why this might happen, and points out that this is a hypothesis at this point. However, he and his research team have some theories.
One theory is that the type of Parkinsons that starts in the intestines is triggered by the gut microbiome, as well as inflammatory states and infections. In addition, the person probably needs to be vulnerable, have some risk genes, Borghammer says. That can include being older, which is a known risk factor for Parkinsons disease, he says.
When it comes to brain-first Parkinsons disease, Borghammer thinks the onset is also probably triggered by certain genes, along with aging.
With both types of Parkinsons disease, the disease process likely starts with bad luck, Borghammer says. In short, the first pathology is simply a random event and, if the person is vulnerable, the pathological process may multiply and spread out of control, he says. Borghammer compares this to the way cancer develops. The first cancer cell is most likely created by random eventsrandom mutationsbut then it multiplies and spreads, he says.
Stage Three Of Parkinsons Disease
Balance is compromised by the inability to make the rapid, automatic and involuntary adjustments necessary to prevent falling, and falls are common at this stage. All other symptoms of PD are also present at this stage, and generally diagnosis is not in doubt at stage three.
Often a physician will diagnose impairments in reflexes at this stage by standing behind the patient and gently pulling the shoulders to determine if the patient has trouble maintaining balance and falls backward . An important clarifying factor of stage three is that the patient is still fully independent in their daily living activities, such as dressing, hygiene, and eating.

Causes Of Parkinson’s Disease
Parkinson’s disease is caused by a loss of nerve cells in part of the brain called the substantia nigra. This;leads to a reduction;in a chemical called dopamine in the brain.
Dopamine plays a vital role in regulating the movement of the body. A reduction in dopamine is responsible for many of the symptoms of Parkinson’s disease.
Exactly what causes the loss of nerve cells is unclear. Most experts think that a combination of genetic and environmental factors is responsible.
What Are The Different Forms Of Parkinsonism
There are three main forms of parkinsonism, as well as other related conditions.
Most people with parkinsonism have idiopathic Parkinsons disease, also known as Parkinsons. Idiopathic means the cause is unknown.
Vascular parkinsonism affects people with restricted blood supply to the brain.;Sometimes people who have had a mild stroke may develop this form of parkinsonism.
Common symptoms include problems with memory, sleep, mood and movement.
Some drugs can cause parkinsonism.
Neuroleptic drugs , which block the action of the chemical dopamine in the brain, are thought to be the biggest cause of drug-induced parkinsonism.
The symptoms of drug-induced parkinsonism tend to stay the same only in rare cases do they progress in the way that Parkinsons symptoms do.
Drug-induced parkinsonism only affects a small number of people, and most will recover within months and often within days or weeks of stopping the drug thats causing it.

Whats The Difference Between Multiple System Atrophy And Parkinsons
Parkinsons and MSA both affect the movement control system and the involuntary autonomic control system and early symptoms can make a differential diagnosis a challenge. MSA, however, tends to progress faster than Parkinsons; balance problems and a stooped posture happen earlier and get worse more quickly with MSA; and autonomic functions such as blood pressure, heart rate, breathing, sweating, bladder function, and sexual problems are more severe in people with MSA.

In Patients Where Parkinsons Disease Starts In The Brain:
The order of symptoms will be opposite patients whose Parkinsons disease starts in the gut, Borghammer says. The pathology probably starts inside the brain and doesn’t really create a lot of symptoms initially, he says. The first clear symptom to emerge is the motor symptoms, signifying that the dopamine system is damaged.
The disease then spreads down in the brainstem, where it can cause sleep issues, he says. Finally, the pathology reaches the peripheral nervous system and causes constipation, urinary problems, and blood pressure problems.
What Causes Tremor
Generally, tremor is caused by a problem in the deep parts of the brain that control movements. ;Most types of tremor have no known cause, although there are some forms that appear to be inherited and run in families.;

Symptoms Of Parkinson’s Disease
These common symptoms of Parkinson’s disease often begin gradually and progress over time:

How Is Tremor Diagnosed
Tremor is diagnosed based on a physical and neurological examination and an individuals medical history. ;During the physical evaluation, a doctor will assess the tremor based on:
- whether the tremor occurs when the muscles are at rest or in action
- the location of the tremor on the body
- the appearance of the tremor .
The doctor will also check other neurological findings such as impaired balance, speech abnormalities, or increased muscle stiffness. ;Blood or urine tests can rule out metabolic causes such as thyroid malfunction and certain medications that can cause tremor. ;These tests may also help to identify contributing causes such as drug interactions, chronic alcoholism, or other conditions or diseases. ;Diagnostic imaging may help determine if the tremor is the result of damage in the brain.
Additional tests may be administered to determine functional limitations such as difficulty with handwriting or the ability to hold a fork or cup. ;Individuals may be asked to perform a series of tasks or exercises such as placing a finger on the tip of their nose or drawing a spiral.
The doctor may order an electromyogram to diagnose muscle or nerve problems. This test measures involuntary muscle activity and muscle response to nerve stimulation.

What Research Is Being Done
